The unit serves a complex patient population requiring advanced clinical assessment, interdisciplinary collaboration, and coordinated discharge planning. As a key member of the nursing leadership team, the Assistant Nurse Manager partners with the Nurse Manager, Clinical Practice Specialist, physicians, rehabilitation services, case management, and interdisciplinary teams to drive excellence in patient outcomes, staff engagement, and operational performance. Position Highlights This off-shift leadership role provides visible operational and clinical leadership during evening hours, supporting frontline staff, patient throughput, quality outcomes, and patient experience. Work Experience Education & Experience - Required Bachelor of Science in Nursing required. If bachelor’s degree is not in nursing, graduate degree must be in nursing. Licensure as a Registered Nurse in the State of Maryland or eligibility for licensure due to compact state agreements outlined through the Maryland Board of Nursing. Three (3) years of previous clinical experience required. Advanced certification (e.g., NE-BC, CEN, PCCN, etc.) required. Incumbents must obtain advanced certification within three (3) years of assuming the position. ACLS, BLS and/or other certifications to match patient populations served. Preferred Education, Experience & Qualifications Master’s degree Previous leadership experience as a charge nurse, clinical coordinator, assistant nurse manager, or equivalent role. Experience in Medical-Surgical, Telemetry, Progressive Care, Neuroscience, or Stroke Nursing. Demonstrated success in staff development, performance improvement, patient experience initiatives, and quality outcomes. Specialty certification such as PCCN, MEDSURG-BC, SCRN (Stroke Certified Registered Nurse), or other relevant certification preferred. The Hospice Home Care Nurse reports to the Team Manager. He/she is responsible for identifying patient/family/facility care needs, and for providing nursing and supportive care for the terminally ill patient and their families. Location: Baltimore County This position supports the residents of Baltimore County from Cockeysville to Towson, including Loch Raven and Overlea. Education: Diploma, ADN and/or BSN from an accredited institution Licensures/Certifications: • Current Maryland State license as a Registered Nurse Experience: Two years of nursing experience, or equivalent combination or education and experience. Skills: • Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection • Strong time management skills • Excellent interpersonal and communication skills • Ability to be patient and family focused • Skill in the use of EMR (Electronic Medical Record) and/or ability to learn Principal Duties and Responsibilities: • Completes initial assessment of patient and family to ascertain home care needs - including physical, emotional, spiritual and mental status. • Prepares and implements the care plan for patient/family unit in collaboration with the Interdisciplinary Group. • Provides direct nursing care to patients in order to maintain highest level of care for the patient in conjunction with the primary physician. • Plans and supervises the delivery of care by the HHA • Cooperates with Clinical Manager in utilization of other team members, outside agencies, and community resources. Updates primary physician when necessary and regularly on a monthly basis. Updates interdisciplinary team at patient care conference • Uses problem oriented charting to ensure continuity of care in a home setting or nursing home as well as inpatient or respite setting. • Teaches family members, facility staff , hired caregivers, patient care skills, including: pain control, medication (RN titrates medication within prescribed range), injections, draws blood for labs, manages IV's, dressing changes, hygiene, nutrition, checking vital signs in accordance with orders from physician, as well as addressing emotional needs. • Ensures that arrangements for equipment necessary in the facility setting are made available. • Attends to patient, family and facility at time of patient’s death. Facility Specific Job Functions • Documents in facility chart • Maintains positive relationships and open communication with facility staff. • Reconciles medications against facility medication list. • Updates facility staff and family of visit schedule • Attends care plan meetings and/or patient care rounds for hospice patients within the facility. • Establish/maintain professional working relationships with a variety of facility staff; ED, DON, medicine techs, aides, nurses (LPNs and RNs), rehab staff, housekeeping, etc. • Provide educational in-services to facility staff both formally and informally. • Participate in marketing and customer service activities with BDMs. Pay Range $66,942.34 - $102,872.41 Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs. As well, the Detox nurse works to educate the patient about his/her condition and to assist the physician and ARNP by the accurate clinical reporting of patient conditions, status and proper clinical actions. The Detox Nurse is involved in nursing assessments at intake and performs necessary discharge nursing actions as well as routine reports needed from clinical for insurance patients. This position is responsible for the proper administration, reconciliation, storage and security of all medications. The Detox Nurse reports to the Chief Nursing Officer. The specific tasks and responsibilities for this position include: Administers prescribed medications, as prescribed by medical team Makes independent decisions within the scope of their license involving patient care Uses sound judgment in the informing of the Chief Nursing Officer/Charge Nurse and the Medical Director of all decisions made and when/how they should be communicated Monitors and records through our Electronic Health System all aspects of the patients care which includes: Nursing assessments during intake Shift Assessments including CIWA and COWS assessments Changes in patient condition and updates on progress; other relevant documentation, that arise with each individual patient Discharge assessments and paperwork, follows Universal Infections Control Precautions using personal protective equipment as required, ensures that direct care staff performs these precautions as well as abides by Infection Control and Exposure policies Reconciles medication at the end of each shift which includes the responsibility of accurately maintaining logs of distribution of controlled substances in accordance to regulation Ensures that during shift change the nurse on duty is properly informed of any information needed to continue high standards of care for all patients.
