As a UAS Assessor, you will conduct in-person assessments for clients to evaluate their eligibility and ongoing needs, contributing to crucial care and support plans. RESPONSIBILITIES: · Conduct UAS assessments for clients to determine their level of care and support needs. · Utilize the Uniform Assessment System (UAS) tool to document assessment results accurately. · Provide compassionate and professional interactions with clients in various community settings. · Submit completed assessments within designated timeframes and maintain detailed records. · Collaborate with other care providers to ensure a comprehensive assessment process. · Attend required training sessions and stay informed of assessment updates.
