Plaza Blvd., Chillicothe, OH 45601 The APP, in collaboration with the Physician, coordinates the health care of primary care patients at this location. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress.
