Shift Monday - Friday, Days Scheduled Weekly Hours 40 POSITION PURPOSE : Following the guidelines of the Illinois Nursing Act, provides professional nursing care for office patients. As a licensed professional, assumes responsibility for the care of individuals and groups through a collegial relationship with a physician or clinician. ESSENTIAL FUNCTIONS : Collects patient health data for physician, enters into the EMR. Analyzes the assessment data in determining nursing diagnoses. Identifies expected outcomes individualized to the patient. Develops a plan of care that identifies interventions to attain the expected outcomes. Implements the interventions identified in the plan of care. Evaluates the patient’s progress toward attainment of outcomes. Assigns ICD-10 codes on charge sheets for billing office and monitors CPT coding to assure proper documentation. May chart on behalf of the provider when in the exam room. Chart note to include name, title, date and time of the entry. Chart must then be signed off on by the provider. Performs other duties as assigned by supervisor. SPECIALIZED KNOW-HOW & REQUIREMENTS: Current Illinois Registered Nurse Licensure required. Previous Medical Office experience preferred. BLS Required within 30 days of hire and must be maintained throughout employment at CGH. Thorough knowledge of medical equipment and instruments helpful. Prior experience in drawing blood is preferred. Demonstrates knowledge necessary to provide care appropriate to any age-related needs of the patients served in the physician office. Demonstrates knowledge of the principles of growth and development appropriate to the patient population served. Able to assess and interpret data about the patient’s status in order to identify each patient’s needs and provide the appropriate care, including age-related care, needed by the pediatric, adolescent and/or geriatric patient group. Shift Days w/ Call Scheduled Weekly Hours 40 POSITION PURPOSE: The Pre-procedural nurse provides care to patients who are preparing for Gastroenterology procedures. The nurse focuses on the preparation for procedures with the patient/family, and the preparation for discharge to home or other extended facilities. This position works closely with the hospital and clinic staff in coordinating patient care. ESSENTIAL FUNCTIONS : Responsible to collaboratively work with the providers for the patient plan of care. Understands the importance of coordinating and collaborating with multiple disciplines to assure best practice and best patient outcome. Responsible for scheduling patients for Gastroenterology procedures, pre-procedural, and post-procedural phone calls. Responsible for consistent patient and family education. Responsible for screening patients for Fast Track colonoscopy. Responsible for completion of documentation to avoid delays on day of procedure. Responsible to assure compliance with the Joint Commission, CMS, and the other regulatory standards regarding care of the Gastroenterology patient, including policy and procedures, monitoring of compliance, and action plan completion. Will assist in Digestive Health Service Line during high acuity times. SPECIALIZED KNOW-HOW & REQUIREMENTS Registered Nursing in State of Illinois. Minimum 1 year CGH GI experience required. Skilled in use of electronic medical record preferred. Ability to write reports and proposals utilizing effective written communication preferred. Shift As Needed Scheduled Weekly Hours 0.1 POSITION PURPOSE : Demonstrates proficiency in administering the fundamentals of the nursing process, contributes to the nursing assessment and the development of the nursing care plan, and participates in the evaluation of nursing care within an assigned unit, department or specialty area. Primary responsibility will be a teaching role. Reports to Department of Nursing Director. ESSENTIAL FUNCTIONS : 1. Perform and document sexual assault medical forensic interviews/exams accurately and timely 2. Perform a complete medical forensic evaluation to include: Head-to-toe medical assessment for trauma, detailed genital/anal examination Forensic specimen collection and supportive documentation Photo documentation of bodily and genital-anal injuries with competent documentation Assess for mandatory reporting status and report when appropriate Maintain chain of custody and adhere to evidence guidelines Ensure patient/medial record confidentiality at all times (HIPPA) Maintain professional standards that do not create conflict of interests with employment or patient care Administer prophylactic medications and emergency contraception Coordinate referrals for appropriate follow-up care as needed 3. Work collaboratively with all departmental and intradepartmental staff.
