About this Job: General Summary of Position The Licensed Practical Nurse (LPN) is a key member of the interdisciplinary team to deliver patient-centered acute nursing care to individuals with a variety of medical issues or who are preparing for or recovering from surgery. The LPN as a collaborative team member in partnership with and under the supervision of the registered nurse. Primary Duties and Responsibilities Organizes prioritizes and cares for groups of patients under the direction of the Registered Nurse. Establishes effective relationships with the patient/ family to facilitate the development a patient centered care plan unique to the individuals care needs. Collaborates with other healthcare team members to implement a plan of care and education to maintain and restore health and prevent complications. Provides patient and family education about measures of health promotion maintenance and restoration to promote comfort. Provides direct nursing care to patients as assigned which includes collect assessment data administration of medications and performs activities of daily living including among others bathing mouth care feeding patients range of motion walking patients and transporting patients in accordance with Nursing Department/Hospital policies and procedures.
