Assesses the patient’s condition, monitors the effect of the care plans on the patient/family health status, informs physician and other team members of changes in the client's needs and updates the plan of treatment, as needed. • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • For NFP Only • Establishes, implements, and revises a plan of care with input from the client and from appropriate multidisciplinary health team members (i.e., social worker, physician) based on goals mutually acceptable to the client. Responsibilities We are actively hiring for an Registered Nurse (RN) Endoscopy. Develop an individualized perioperative patient plan of care by implementing: P rotecting the patient from injury caused by extraneous objects and chemical, electrical, mechanical, and thermal sources. D emonstrating safe operation of equipment and machinery and follows procedures for reporting and correcting an unsafe situation. S peak ing up with safety concerns acting as the patients advocate. Handling specimens according to facility policy and procedures. Adher ing to standard precautions including the use of personal protective equipment. Complying with Universal Protocol and facility site validation and time-out policy C ommunicating the patient’s current status to relevant parties throughout the continuum of care. A dministering medications safely and correctly using the 8 rights of medication administration. P racticing and monitoring aseptic technique . Implement ing the plan of care including the following interventions: M anaging patient recovery from anesthesia – Moderate sedation and Monitored anesthesia care . Assess ing patient status at intervals in accordance with facility policies and procedures . P roviding nursing care and appropriate monitoring in accordance with policies and procedures . P erforming interventions to maintain the patient’s tissue perfusion at or above baseline levels. Ensuring scopes are prepared for use in accordance with policies and procedures and manufacturer’s instructions for use . Maintain ing endoscopy instruments and accessories in working order. Troubleshooting and recognizing malfunctions . P erforming interventions to maintain the status of the patient’s genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, and neurological systems and fluid, electrolyte, and acid-base balances at or above baseline levels. P erforming interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period. P erforming interventions to protect the patient from infection. A ssessing the knowledge level of the patient or designated support person. P roviding education regarding the expected psychosocial response, nutritional management, medication management, pain management, and expected responses to the procedure. I nvolving the patient or designated support person actively in decisions affecting his or her perioperative plan of care and the rehabilitation process. P rotecting the patient’s rights, dignity, and privacy. P roviding age-specific, culturally competent, ethical care within legal standards of practice . Evaluate the patient’s progress toward attaining outcomes. Transfer care as applicable. Provide hand-off report to other teammates that are accurate, concise and pertinent to the patient’s condition and response to care. Delegate tasks according to the state Nurse Practice Act (if applicable), state board of nursing or professional registration requirements, state laws, and job descriptions . Document episode of care accurately, timely, completely, and legibly. Demonstrate effective communication and collaboration skills with members of the team and other stakeholders. Serve as a member of a multidisciplinary team in planning patient care. Participate in quality review and performance improvement projects. Participate in the performance appraisal process. Use problem-solving and conflict resolution skills to foster effective work relationships. Maintain required competencies. Pursue professional growth. Act as a patient advocate and maintain privacy and confidentiality of individuals and health information. Comply with al SCA and facility policies and procedures, regulatory and accreditation standards and professional guidelines when providing patient care. Actively participate in team meetings, trainings, and daily safety huddles. Demonstrate fiscal responsibility. Perform all other duties as assigned. Qualifications Graduate of an a ccredited s chool of n ursing . O ne year of GI, OR and /or medical / surgical nursing experience preferred . Current state nursing licensure to practice in good standings . Successful completion of Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) within 90 of employment or documentation of current BLS/BCLS certification. Successful completion of Advanced Cardiovascular Life Support (ACLS) Course within 90 days of employment or documentation of current ACLS certification. Successful completion of Pediatric Advance Life support (PALS) course is required when pediatric care is included in the facility scope of services within 90 days of employment or documentation of current PALS certification. Specialty certification in GI Nursing is preferred. USD $44.00/Hr. USD $51.00/Hr.

Nurse Practitioner Physician Assistant Job Description Position Summary: Conducts comprehensive health assessments, demonstrating a high level of autonomy and expert skill in the diagnosis and treatment of complex responses of individual, families, and communities to actual or potential health problems. Integrates education, research and management, leadership and consultation into clinical role and functions in collegial relationships with nursing peers, physicians, professionals and others who influence the health care environment. Provides health care in accordance with standards of practice as delineated by state practice guidelines and national board certification organizations. Major Responsibilities: Provides diagnostic and assessment services. Secures a health history from patient and/or family, records findings, and evaluates them. Performs a complete in–depth physical examination. Orders/performs pertinent diagnostic tests based on age and history. Analyzes data collected to determine health status and identify differential diagnoses based on history, physical exam, and clinical findings. Partners with physicians regarding proposed plan of care. Formulates this plan of care with the patient. Manages therapeutic regime as outlined by established protocols for patients with acute illnesses. (Protocols are guidelines concerning patient care that are established between the physician and nurse practitioner. The guidelines outline subjective and objective findings, treatment plan and follow–up). Reassesses and modifies plan as necessary to achieve medical and health goals. Confers with physician for clinical direction as outlined by protocols. Documents patient care outcomes to determine effectiveness of plan of care. Communicates outcomes with physician. Position Qualifications: License/Certification/Education: Required: A Bachelor’s degree and Graduation from an accredited Physician Assistant program. Current Massachusetts license, issued by the Physician Assistant Board, and current NCCPA certification. A Master’s degree and graduation from an accredited Nurse Practitioner program. Preferably ACNP/AGACNP Acute Care/Critical Care clinical experience required Board Certification by the American Nurses Credentialing Center (ANCC) or the American Association of Nurse Practitioners (AANP).
