UNIT DESCRIPTION: 8 North East and West CPCU makes up a 68-bed unit cardiac step-down unit that provides an intermediate level of care to patients that typically have a cardiac related diagnosis such as coronary artery disease (CAD), congestive heart failure (CHF), myocardial infarction (MI) and heart arrhythmias such as atrial fibrillation (A-fib) and ventricular tachycardia (V-Tach). We also provide pre and post procedure care to patients that have undergone cardiac procedures, such as catheterization/angioplasty, stents, ablations, and placement of pacemakers, and defibrillators. Our nurses receive specialized training in arterial sheath removal and Tikosyn administration and are ACLS certified. All patients are continuously monitored by telemetry. Monitors and evaluates nursing care given, makes rounds throughout hospital.Provides supervision to staff nurses and psychiatric care specialist on his/her designated shift, including disciplinary action. Participates with the Nurse Unit Manager and other nursing managers in the development of therapeutic programs for the unit and particularly for his/her designated shift. Ensures auditing charts for completion, quality and meeting of standard Responsible for ensuring that all admission during shift are handled appropriately, following the admission policies and procedures. Assists/oversees restraints or use of seclusion in an emergency situation. Notifying the patient’s physician for orders as soon as possible within the guidelines of Seclusion and Restraint Policy. Reviews policy and procedures on a regular basis. Directs activities in all emergency situations, including fire, internal and external disasters, and environmental hazards (tornados) according to procedures. Reviews intake information as needed and collaborates with the Intake Referral Coordinator to ensure a timely referral process. Investigates unusual incidents/complaints from patients, personnel and other departments and reports to the Assistant Chief Nursing Officer. About this unit 8 South Neuro is a 52-bed progressive/medical surgical, comprehensive Stroke certified unit that offers a solid foundation in nursing for new grads. The unit has a focus on acute strokes, neurosurgical interventions, such as craniotomies, spine procedures and VP shunts. In addition, a 4-bed epilepsy monitoring unit. Performs discharge planning coordination/referral by making appropriate referrals to social services, ancillary departments, outpatient case management, DME, post-acute placement, and other outside agencies per Standard Operating Procedure (SOP). Acts as a liaison by collaborating and communicating daily with the physician, patient, family, nursing, and other members of the healthcare team. Actively participates in clinical case review/rounds with the interdisciplinary team. Documents in the electronic medical record (EMR): assessment, plans, interventions, barriers, and reassessments to facilitate discharges and/or transitions, m anages anticipated discharge date and e nsures all pertinent information is transferred to post-acute agency. Identifies barriers early in the patient’s stay, formulating a plan with the patient, family, internal and external members of the healthcare team, payers, and community resources. Identifies and reports avoidable day/variances and/or service delays from established plan of care to leadership. Represents the integrated care management department on various teams and performance outcomes committees and projects. Ensures patients follow up appointment with PCP has been made prior to discharge. Maintains effective operations by following policies and procedures.
