Job Description The Department of Orthopaedic Surgery & Rehabilitation Medicine at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner, Joint Reconstruction. The successful candidate will: Support the outpatient clinic operations for Joint Reconstruction Service physicians in reliable, timely, and efficient manner. Provide consultation to clinical staff regarding the nursing management of Joint Reconstruction Service patients. Work collaboratively with other members of the Department, other health care professionals and physicians to enhance the quality of patient care outcomes. Utilize evidence-based practice data to identify trends and improve outcomes. Collaborate with healthcare team to ensure efficient patient admission, transfer, and discharge. Participate in QAPI projects. Support the delivery of quality patient care by holding self and staff accountable to meeting or exceeding established standards. Act as a clinical resource for novice staff members as well as assisting experienced staff in problem solving skills for complex patient care situations. Attend weekly grand rounds and case presentations to build knowledge base. Assist residents on the service and in the Division of Orthopaedic Surgery with processing paperwork and patient logistics. Compost clear, succinct, thorough, and appropriate clinic notes, utilizing templates where possible to improve efficiency. Arrive on time to clinic and stay until all necessary and reasonable tasks have been completed. Follow and anticipate attending physician instructions, especially for common tasks. Continually seek to increase knowledge and understanding of the Orthopaedic joint reconstruction sub-specialty. Arrange consults when appropriate. Use macro templates in EHR systems. Create progress and discharge notes, where appropriate, for each patient seen. Use service recovery strategies within scope of responsibility to resolve immediate patient concerns. Participate in the development, implementation and evaluation of clinically based education programs. Participate in specialty associations for the enhancement of continuing education. Perform other tasks for the department and practice as needed. Demonstrate flexibility to adapt to new and evolving challenges. Recognize changes in clinical situations and act in a timely manner. Adhere to the hospital and medical group policies and procedures. Be a role model for other members of the team. while modeling the WE CARE values of the institution. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information. Incumbents attempt to encourage and support patients who may be despondent or apprehensive, and regularly communicate with clinical staff to convey information on patients’ physical conditions and responses to treatment. On a daily bases, review all laboratory and radiology reports to identify abnormal results for urgent follow up; ensure that patients are contacted to arrange additional care and that their primary providers are notified. Participate in and provide departmental and Family Medicine educational activities to medial students and allied health professional students. Participate in hospital and departmental health fairs, community education events and speak to community groups, when requested. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Notifies RN if rounding has not been completed. • Utilizes Vocera system/Mount Sinai phone according to sign in/sign out protocol where activated. • Performs other duties as assigned, including generating reports • Is open and responsive to the diverse backgrounds and experience of other people and promotes an environment that is sensitive to cultural diversity. • Respects and values each patient’s unique, individual needs, beliefs, and diversity and incorporates these into delivery of care. • Greets visitors and answers telephones promptly and courteously. Escorts visitors to appropriate area, when necessary. Provides information as appropriate. • Answers Patient Call System and refers calls to appropriate staff. • Maintains a safe, clean and functional environment, including removing trash and bringing physical plant and equipment problems to the attention of the managers. • Transports patients to stretchers or wheelchairs, as required • Delivers and collects trays and bedside, delivers flowers to patients. • Strips units/cubicles • Performs emergency clean-up • Performs emergency transport • Performs discharge bed making • Transport specimens. • Updates communication board/discharge log • Assembles medical records • Updates Admission/Transfer/Discharge System/Processes • Maintains supplies and care of equipment. Communication/Relationships • Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others. • Displays courtesy, tact and patience during interactions with all members of the hospital staff and extended community.
