Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. As a member of the Medical Staff the APN demonstrates standards of professional practice that includes ethical decision making, job activities related to quality assessment/improvement, self- evaluation, leadership, and research. The APN is self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency.In this role, with a specialized focus on prenatal patients, the APN will utilize critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of Medical/Psychiatric conditions/diseases. The APN practice will promote maternal and fetal wellness through patient education, preventative care and participation in research initiatives. Functioning both autonomously and as part of an interdisciplinary team, the APN serves as a clinical resource and consultant, supporting coordinated care across the prenatal continuum. Responsibilities What You’ll Do Collects comprehensive patient health data/history including past and present medical, surgical, family and social history. Diagnostic tests and procedures relevant to the patient’s status are collected and interpreted. Completes the medication reconciliation process by documenting the current list of medications (including over the counter and nutritional supplements) taken by patient at each visit. Conducts clinical rounds on F-Green, Labor & Delivery, Outpatient Pediatrics and OB/GYN units. Proactively identifies patients with elevated maternal depression or psychiatric risk factors and ensures comprehensive pre- discharge engagement. Provides medical and psychiatric care to patients on the Inpatient unit, in the ED, on medical floors and/or in the Outpatient Clinics. Provides education to patients and families to maximize their utilization of insurance, resources, and physician recommended care practices to improve overall quality of life. Monitors intended and potential side effects of medications and treatments ordered and will offer appropriate, accepted interventions. Provides monitoring and continuity of care between departments/services; collaborates with Clinical Care Coordinators, Patient Navigators, and medical staff to coordinate continuity of care. Qualifications What You’ll Bring Required: A current Registered/Professional Nurse license and registration in the State of New Jersey is required. Completion of a master’s degree program, MSN – Psychiatric Mental Health Nurse Practitioner (MSN-PMHNP) or Post Master’s Degree Certification and eligible forcertification by a national certifying body. Performs and documents an appropriate comprehensive physical assessment of patients which will include a review of systems, evaluation of manifestations and toxicities related to medical conditions and anticoagulation management. In collaboration with the physician, critically analyzes the assessment data/diagnostic tests and determines a diagnosis. In addition, develops a comprehensive plan of care that includes interventions of treatments to attain outcomes. Evaluates patient progress in attaining expected treatment outcomes. Monitors for signs and symptoms related to disease progression/recurrence. Appropriately manages medical and anticoagulation management emergencies according to protocol/policy. Prescribes and/or orders medications. Obtains appropriate consents for surgery and procedures. Ensures completion of all needed pre-operative tests, forms, and documents. Documents assessments, prescribing, plan, modifications to plan, and evaluation of care in the EMR. Monday - Friday and Every Other Weekend required. Education: High school education required. Valid Certified Nursing Assistant certification required. Competencies: Supports the medical/clinical needs of the resident by transporting and timely preparation for rehabilitation therapy, medical tests, and consultation/physician appointment. Collects timely and accurate specimens, vital signs, weights and measurements. Reports findings to nurse or other appropriate clinicians. Demonstrates receptiveness to new ideas, and responsiveness to constructive criticism. Contributes to the overall care, concern and comfort of all residents on the wings. Assist unit as deemed necessary. Appropriately encourages residents to participate in self-care of ADLs as part of their rehabilitation and/or maintenance of abilities. Assists in the maintenance and enhancement of the residents personal space and environment by making beds, maintaining room temperature, assuring personal preferences of stimulation are offered/supplied i.e.: music, radio, snoezlen, activity box. Assures all equipment, furnishings and space is clean and in good repair. Provides accurate and appropriate feedback, communication and evaluation to resident and family regarding care provided by CNAs. Refers clinical/medical questions to nurse or appropriate clinicians. Follows standard Precautions Guidelines for Infection Control. Reports any communicable or infectious disease to the shift Supervisor. Follows established isolation precautions and procedures. Provides personal care to residents with a focus on resident preferences in regard to how and when care is provided for meal assistance, feeding, washing, bathing, mouth care, dressing, hair care, nail care, shaving and bed/chair positioning. Follows the plan of care in handheld PDA assigned residents. Provides timely and accurate documentation. Helps to improve or maintain resident’s level of functioning and health status through proactive and timely assistance with bowel and bladder functions. Recording intake and output, ambulation, transfer, range of motion, use of assistive devices/splints. Appropriate use of restraints, per policy. Provides comfort to resident throughout the day with a focus on resident preferences, including regular repositioning in bed or chair, changes of clothes or bedding when soiled or wet and pain management through key times rounding and answering call bells within standard time. Checks and documents proper functioning and use of specialty mattress. Assures nurse call bell is always within reach and call bells are answered promptly. Observes and reports all unusual changes, occurrences, including physical or emotional status, functioning, behavior; including falls, skin breakdown, pressure wounds, complaints, grievances and clinical needs. Reviews and updates Personal Care Records. Supports the resident and family during transitions of admission, transfer or discharge including orientation and welcome to unit/room, packing/unpacking of personal belongings. (ESSENTIAL; DAILY) Encourages and assists resident in participation of activities and social programs, i.e.: parties, church services, outdoor programs, snoezlen. Regularly uses resident activity boxes. Offers timely and regular transport to activities. Demonstrates consistent reliability/dependability through attendance and punctuality. Adheres to Christian Health’s Code of Ethical and Legal Behavior, including completion of specific training and educational programs as deemed necessary for the position. Overview About the Role The Clinic Nurse Manager is an experienced clinical leader responsible for managing assigned areas and multiple services within the assigned practice, supervising assigned patient care personnel during clinic operations. The Clinic Nurse Manager is responsible to ensure the safe and effective delivery of patient care and clinical outcomes, compliance with regulatory, administrative, and clinical standards, policies, and procedures. The Clinic Nurse Manager is seen as an expert in clinical practice topics and is proficient in personnel management and directs care to meet the unit demands in an efficient and cost-effective manner, as well as assisting with clinic budget management. This position reports to the Administrative Director of the practice and the Executive Director Clinical Operations. Responsibilities What You’ll Do Maintains nursing and clinical operations by initiating, coordinating, and enforcing program, operational and personnel policies and procedures within the assigned practice in the Ambulatory Care Center. “Owns” clinic operations and exercises broad oversight for clinic patient throughput, provider performance, and clinical and/or support services. Maintains accountability for daily staffing patterns and appropriate skill mix to support the practice’s volume and acuity. Qualifications What You’ll Bring Required: Licensed as a Registered Nurse in the State of New Jersey. Bachelor’s Degree in Nursing (BSN) required, a Master’s Degree (MSN) preferred. A minimum of five (5) years of nursing experience required. BLS (CPR) is required. Certification as an Ambulatory Nurse (RN-BC) is desirable and preferred within two (2) years of hire. Must have the ability to lead a team and maintain maximum effectiveness and efficiency. Strong oral and written communication skills, and exceptional customer service skills required.
