Job Overview Fairview has an immediate opening for a Licensed Practical Nurse to join our Dermatology team in Bloomington, MN This is a 0.9 FTE ( 72 hours per two week pay period), benefits-eligible opening. Clinic Hours of Operation are Monday – Thursday: 7:00AM – 6:00PM and Friday: 7:00AM – 5:00PM. The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines. Responsibilities Collects data for patient assessments. Involves the patient, family, and other health team members in gathering relevant data. Document’s data collected in the medical record. Reports unusual data or changes in patient condition to the RN, or physician/authorized practitioner in a timely manner. Department Overview Our Minneapolis 7th floor is a 33-bed pediatric unit that provides care to infants, children and young adults in the areas of hematology, oncology, general medical/surgical care and patients needing behavioral crisis monitoring. Our team of dedicated professionals specializes in care of patients with hematology and oncology diseases, including new diagnosis, transfusions, chemotherapy administration, central line maintenance and end-of-life care. Our 7th floor staff also care for general medical/surgical patients, particularly those with non-infectious illnesses in order to maintain a clean environment for our patients with weakened immune systems. We utilize three inpatient rooms to support patients who need behavioral crisis monitoring. Our patients have an average length of stay ranging from a few days up to months depending on the diagnosis, monitoring and treatment the patient is receiving. Position Summary This position provides specialized medical consultation, evaluation, and care for children and adolescents with concerns related to child maltreatment. Key responsibilities include: * Providing phone consultation and triage for caregivers, medical providers, and community professionals. * Conducting medical evaluations, including video-recorded histories, for suspected victims of child maltreatment. * Collecting physical evidence in acute cases of sexual assault or abuse involving children and adolescents. * Providing medical evaluations and ongoing care for adolescents in youth shelter settings. * Coordinating care with a multidisciplinary team. * Occasionally providing testimony in legal proceedings. Pre and Post Op Care NAs will assist in all areas of the unit with patient ADLs, obtaining weight and vital signs from patients, and assisting with set up, cleaning, and stocking rooms. This position has varying start times, as listed below: 5:30am-2pm (Pre Op Start Time) 8am-4:30pm (SDS) 10:00am-6:30pm (Main) 2pm-10:30pm (Main) Job Responsibilities The Nursing Assistant is responsible for assisting the professional nurse/caregiver in the delivery of patient care, transporting patient and/or equipment, setting up and maintaining patient rooms, and the reordering and stocking of supplies. The Nursing Assistant does not work autonomously but works under the direction of a professional nurse. The Nursing Assistant may be given a patient care assignment based on unit needs and the discretion of the manager/charge nurse. Provides basic patient care by performing routine duties to include: Personal grooming, bathing, toileting, Feeding, Skin care needs (clean and sterile dressing changes), applying ointments/creams, Oral hygiene, Specimen collection/testing, Position/ambulating, Changing linens, Promptly answers call lights. Assists the nurse in monitoring patient condition. Accurately takes and records weights, input/output, blood pressure, temperature, pulse rates, respiration, oxygen saturation. Places patient on/off monitors or equipment. Reports unusual findings to the RN in a timely manner. Assists with admissions, transfers, and discharge of patients. Orients patient and their family to the environment. Assures the patient’s belongings are accounted for, documented upon admission, transfer and discharge. Transports patients, equipment, specimen and supplies. Assures patients get to appointments on time. Uses transfer belt when assisting patients with transfers and/or ambulation. Cleans sets up, and maintains rooms and equipment. Orders and stocks supplies. Accurately documents tasks performed on appropriate form. Performs other tasks as needed. Required Qualifications License/Certification/Registration Basic Life Support (BLS for Healthcare Providers) certification required. Ontrack participants much be able to obtain Basic Life Support for Healthcare Providers certification upon completion of the OnTrack program. AND ONE or more of the following: Successful completion of Fairview’s OnTrack Nursing Assistant training program or another Fairview sponsored Nursing Assistant program. Or Six months of recent direct care experience in a long term, home care or acute care setting. (Appropriate roles may include, Nursing Assistant, PCA, Resident Assistant or Emergency Room Technician) Or Current enrollment in an LPN, RN or Nurse Practitioner program, and completion of one (1) semester of clinical rotation within the program. Or Current certification on the Minnesota Nursing Assistant Registry upon hire. This requirement does not need to be maintained throughout employment and is not required if subsequently transferring into another Nursing Assistant or Nursing Station Tech or Nursing Station Attendant position.
