Location: Parkland: 16th FLR BLK 400 **NIGHT SHIFT** Full Time Position Primary Purpose A Registered Nurse (RN) with > 2 years of experience. The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, that focuses on a particular patient population(s)/specific health conditions, by the health care team. Consults with other team members as required, notifies providers to report changes in patient condition and implements orders in a timely manner. Must be able to manage an adult ICU patient with neurological, pulmonary, cardiovascular, gastrointestinal, genitourinary disorders/conditions. Must provide safe medical administration using the patient's six (6) rights, provide maintenance and management of IV access, must be able to perform POC monitoring, demonstrate safe infection control practices and CRRT for the adult ICU patient. Supports pain management as appropriate for each individual patient, provides emergency resuscitation care of an adult and provides an individualized plan of care. Maintains and implements knowledge of applicable rules, regulations, policies, laws and guidelines that impact or govern nursing practice, and ensures the hospital and Parkland are in compliance. Develops effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Maintains and updates knowledge of sanitation, infection control, safety, supplies, equipment, appropriate utilization of supplies and materials. Completes appropriate documentation. Maintains accurate and timely entries and notations in patient care records and hospital documents. Ensures that all records and documents meet the standards, goals, and objectives of the Adult ICU/Critical Care department and Parkland. Works with other staff in providing direction to non-nursing staff. Participates in information exchanges with nursing and other Parkland staff that support the standards, missions, goals and objectives of the Adult ICU/Critical Care department and Parkland. Seeks and utilizes direction from more experience staff and team members as needed. Provides appropriate hand off any time care is relinquished. Serves as Charge Nurse on rotation as necessary. Serves as a preceptor or trainer to newer nursing staff members. Prepares and conducts an in-service educational program for the Adult ICU/Critical Care department at least once per year. Identifies ways to improve work process and improve patient and customer satisfaction. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall goals of the Adult ICU/Critical Care department and Parkland. May serve on various interdisciplinary committees as assigned. Obtains informed consent from patients prior to the delivery of services per organization protocols. • Supports the clinician by performing lab testing and vital signs appropriate to the patient and notifying the clinician of special concerns pertinent to the visit. • Reviews test results with patients and when appropriate, educates patients with abnormal results regarding the nature of the problem, method and treatments prescribed, and instructions and information about proper follow-up, including referral to appropriate medical, financial, and social service resources when necessary. • Consistently utilizes an empathic approach to patient care delivery. • Provides compassionate care to patients using a holistic approach ensuring that patients’ physical, mental, social and emotional needs are addressed. • Provides contraceptive supplies, prescribed medications, and intramuscular injections as ordered by the clinician per health center protocols. Provides specific instructions on each patient’s selected birth control method. • Triage phone calls to clinicians as appropriate. • Reviews electronic practice management (EPM) and electronic health records (EHR) systems for any alerts for outstanding follow-up needs and advise patients of their follow-up status or discuss the clinician’s recommendations with the patient at each visit. • Maintains an awareness of clinic flow and visit cycle times and contributes to the efficient delivery of care by working with a sense of urgency to respect the time of patients and increase access to services. • Maintains discretion and patient confidentiality at all times, using appropriate guidelines on release of information. • Contributes to a culture of continuous quality improvement by reporting incidents timely and completing incident reports. • Understands and complies with regulations from all governing entities and health center protocols including but not limited to Patient Rights, Child Abuse identification and reporting, infection control, laboratory protocols, emergency evacuation, risk management, and Occupational Safety and Health Administration (OSHA) safety standards. • Performs the following front office duties as assigned: o Greets and directs guests appropriately and in a courteous manner that projects a positive image of PPGT. o Receives and directs phone calls, schedule appointments, check-in patients, obtain necessary patient information to file and update patient records, and ensure all forms and consents are completed by patients o Supports the check in or check out areas. o Places reminder phone calls for scheduled appointments. o Visually inspects visitors/patients and packages entering and leaving the health center. o Observes security cameras and grounds, secures building at end of shift. o Completes daily health center closing procedures such as preparing deposits and balances cash drawer. Location: Acute Care Hospital Tower Shift- 12hr Days (7am-7pm) Work Hours: This program is a 7 day a week service, rotating every other weekend and rotating holidays Employment Type: Full Time Hospital at Home is an innovative program that combines in-home with remote monitoring, offering patients timely, hospital-level care in their homes. Though the list of diagnoses enrolled in Hospital at Home continues to expand, common disease states include acute pneumonia, cellulitis, COVID-19, urinary tract infection, dehydration, or exacerbations of chronic conditions, such as heart failure or chronic obstructive pulmonary disease. For many patients, the program provides an alternative to the traditional hospital setting and results in improved patient outcomes and satisfaction. The Hospital at Home team is looking for motivated nurses with critical thinking skills, acute care experience, and an interest in serving the Parkland community. Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate acute patient care delivery in the patient home. Using the nursing process, the RN assesses the patient in their environment, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and providing healthcare in the home environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition, and the competencies of the employee. Minimum Specifications Education Must be a graduate from an accredited school of nursing. Prefer Bachelor of Science in Nursing. Experience Must have two years of nursing experience in the acute care setting. Prefer experience in the home environment.
