This individual is a member of the treatment team providing professional level of nursing care to patients under minimum supervision and consultation. This is a complex position which directs and guides activities on the unit and assignments of ancillary personnel assigned to unit and shift. Responds to medical and psychiatric concomitants in treatment and develops nursing interventions that are appropriate for the situation. Promotes patient's independence by establishing patient care goals, teaching patient to understand condition, medications, and self-care requirements, answers patient questions. Cross trains in medication administration in order to complete safe medication passes following standards of care per the Board of Nursing. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations. Resolves patient problems and needs by utilizing multidisciplinary team strategies, including de-escalation techniques. Protects patients and employees by adhering to: infection-control policies and protocols, medication administration and storage procedures, and controlled substance handling. Maintains continuity among teams by documenting and communicating actions, irregularities, and continuing needs. Maintains patient confidence and protects operations by keeping information confidential. Ensures compliance with Patient Privancy, HIPPA and Joint Commission Standards. All employees with direct patient contact are responsible for actively engaging in the practice of, monitoring and enforcing compliance to hand hygiene. Assesses (and reassesses), plans, implements, and evaluates nursing interventions/plans of care/patient goals. Provides patient care based on standards of care, standards of practice, protocols, and established policies and procedures. Respects each patient as an individual; provides care that fosters the patient’s sense of dignity and positive self-regard. Provides patient/family teaching based on identified learning needs, readiness to learn, and barriers to learning. Collaboratively plans and prepares patient/family for discharge incorporating support systems/resources as necessary. Appropriately delegates and supervises nursing interventions performed by others. Appropriately prioritizes patient care (e.g. seeks assistance as needed from appropriate resources, reassigns personnel as necessary, recognizes need for emergency intervention, etc.). Provides thorough, timely, and accurate documentation of all pertinent data, therapeutic interventions and patient responses according to established standards. Provides timely and thorough report on assigned patients to other health care team members to ensure integration of services and continuity of patient care. Promotes learning for colleagues and self (e.g. functions as preceptor, instructor, trainer, attends professional development programs, etc.). Proactively identifies risk to patient safety and identifies opportunities to reduce medical/health care errors (i.e. appropriate reporting of errors/incidents). Identifies ethical issues in care and develops plan to address. Safely administers medications and intravenous therapy in accordance with physician’s orders, protocols, policies and procedures. Assesses patient pain level upon admission and as necessary, depending upon diagnosis utilizing the numerical pain scale. Reassesses effectiveness of pain control and notifies physicians as needed, as well as, documents assessment and patients’ response to medication. Responsible for obtaining supporting information and completing required documentation related to medication reconciliation for use by the Attending MD. Collaborates with surgeon, anesthesiologist, clinical coordinator and surgical technicians to assure the patients’ well being during surgical intervention. Maintains sterile fields and directs the correct care, handling, and sterilization of sterile supplies and instruments. Identifies patients correctly by checking patient identification bands and consent forms as well as verifying operative procedure and site before taking patients to the operating room. Applies principles of aseptic practice, oversees the selection and preparation of supplies, equipment and instrumentation to all surgical cases. Applies principles of body mechanics and adapts them to various situations during positioning and transfer of patients. Assures that all techniques and practices carried out during the surgical procedure are accurate and reflect standards of perioperative practice. Maintains sponge, needle and instrument counts according to policies and procedures, including notifying the surgeon of the result of the counts. Identifies, labels, cares for and handles all specimens and cultures according to established policy and procedure. Occasionally stoops (bending the body downward and forward by bending the spine at the waist), and kneels (bending the legs at knee to come to rest on knee or knees). Qualifications Qualifications : Current MA RN License required. ACLS upon hire OR must hold an active BLS card and obtain AHA ACLS within 60 days of hire. Recent OR experience required. CNOR preferred. Excellent communication and critical thinking skills required. Must take call per unit rotation.
