Description The Assistant Nurse Manager for CSICU is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. The Citadel at Kingsbridge Rehabilitation & Nursing Center is currently seeking Full-Time CNA's for day (7am-3pm) evening (3pm-11pm) and night (11pm-7am) shifts to join our dynamic organization. OVERVIEW: CNA is responsible for providing direct resident care including clinical, personal, nutritional and restorative care. RESPONSIBILITIES: · Help patients use the restroom · Clean and bathe patients or residents · Maintain patients' hygiene and change bedding · Turn, reposition, and move patients between wheelchairs, seats, and beds · Serve meals and help patients eat their food · Measure vital signs like blood pressure and temperature · Listen to patient’s health concerns and report details to a RN or Doctor REQUIREMENTS: · Valid CNA License in the State of New York · CPR/BLS certified · Minimum 1 year nursing experience in a Long term care Facility.
