Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required? Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. Summary The MH Registered Nurse - Ambulatory Care role provides high quality care to patients and families calling or presenting for care through assessment, planning, implementation, and evaluation, and performs a wide variety of patient care services to assist physicians/advanced practice practitioners in delivering excellent care to patients of the practice. MH RN II: This level is competent in their clinical role and has demonstrated the ability to care for most patients assigned. Though not able to care for every patient type within their area of hire, demonstrates how to obtain the appropriate resources to develop required skills and knowledge over time. This level included automatic advancement after 1 year as a RN I – must be in good standing and meets RN I requirements. Required Minimum Knowledge, Skills, And Abilities (KSAs) Education: Graduate of an accredited School of Nursing required; BSN preferred. License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Current BLS certification required or must obtain within 30 days of start date. Experience: One year of recent RN experience required. Automatic advancement from RN I level if all onboarding requirements are complete, including the New Grad Residency (where available).
