Possess and maintain valid CPR certification while employed in a clinical role, preferred Must be a licensed driver who can travel to all business locations Meet the regulations and requirements of the state(s) in which program provides services Responsibilities: As a Palliative Care Nurse Practitioner, you will: Develop and implement clinical plans of care for patients with chronic and terminal illnesses Perform patient assessment, history and physical; recommendations on aggressive symptom-management Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teaching Maintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basis Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family May provide support at time of death Prescribe medication as permitted by the state and/or the State Board of Nursing Participate in continum of care for patients that are hospice appropriate and ongoing education programs. The ED plays a significant role in the hospital’s Stroke, AMI and trauma programs. There is also a specialized area for adult and pediatric mental health evaluations. RN - Admissions Case Coordinator Position Summary The Admission Case Manager is an active member of the Patient Care Management and ED team. The role combines every function of case management, from Access through Discharge planning, and post-discharge from the ED. The case manager serves, manages, and supports internal and external customers, communicates appropriately using good interpersonal skills, participates in performance improvement initiatives and programs, participates as a team member, and is accountable for own work responsibilities. Three core responsibilities of the Admission Case Manager are to provide: Consultation and direction for physicians about the appropriate determination of the level of care, specifically Observation vs. Inpatient status.
