Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Collaborates with practice providers and all other members of the clinical support team in planning for the clinical support of providers in practice. IMPLEMENTATION / EVALUATION Provides individualized nursing care which responds to the special needs of the patient population and demonstrates and understanding of the treatment needs required for patients. Performs nursing intake process according to the standards in the nursing policies and procedure manual. Performs nursing activities as ordered by the physician, including venipunctures and electrocardiograms. Administers medications and thoroughly documents in the medical record. Maintains current knowledge of pharmacology and assure medications are current. Instructs patients on specific diagnostic procedures (specimen collection, etc. ) Assists as needed with diagnostic procedures and treatments. Maintains knowledge about the operation and care of biomedical equipment in the practice . Responds to the emotional needs of patients by demonstrating a sensitivity to their emotional state by relating to patients in an appropriate manner. Refers to appropriate members of the health team as indicated. Responds appropriately to emergencies and maintains knowledge of emergency procedures and supplies available. Complies with all ACC and Medical Center policies and procedures for infection control, safety, administrative, and clinical practice. Complies with activities mandated by The Joint Commission /Title 22. COORDINATION / CONTINUITY / COMMUNICATION Organizes nursing care based on the needs of patients, while maintaining the effective flow of patients through the clinic. Adjusts the clinical support activities to reflect changes in priorities during practice sessions. Maintains open communication and facilitates continuity of care for patients and/or family with members of the health care team through phone contact, verbal interaction and/or written reports. Utilizes electronic healthcare information (i.e. labs, appointments) to contribute to patient care. Assists with coordinating diagnostic testing. Maintains accurate follow-up on results and contacts patient with any significant result requiring treatment ordered by the Physician. Answer Nursing telephone line messages. Responds appropriately within the scope of Nursing and recommends treatments under the supervision of a physician and/or use of standardized protocols. Maintains open communication with ancillary departments such as pharmacy, sterile processing, EKG and the laboratory to problem- solve issues. Actively participates in all staff meetings. Demonstrates knowledge of and effective use of communication to resolve questions, problems, concerns, and/or issues. Demonstrates courtesy, common sense and follow-through in all telephone communications, maintaining the Telephone Standards as established by the Medical Center staff. ADVOCACY Provides patients and/or their families with the necessary information to make informed decisions utilizing available resources and makes referrals appropriately. Informs patients/family about services available at UCSF and in the community. Informs patients of the Patient Bill of Rights and encourages involvement in their own health care. _________ PATIENT EDUCATION Participates in identifying the educational needs of the all patient populations. Provides and reinforces patient health teaching. Evaluates the effectiveness of health teaching by obtaining feedback from patients. Participates in the review and/or revision of established educational materials. Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments. Assists as needed with diagnostic and therapeutic procedures and treatments. Uses appliances and equipment correctly and maintains safe practice. Obtains specimens and performs Point of Care Testing as directed. Performs QA/QC on CLIA waived tests and reports any tests out of QA range to supervisor. Completes annual competency testing as specified by the individual practice. Assists patients with limited mobility to transfer from wheelchair to exam table, chair, or toilet. Qualifications Required Qualifications: Minimum of one-year recent experience as a Registered Nurse in an ambulatory practice This position requires flexibility to orient and work at all UCSF Medical Center locations Preferred Qualifications Strongly preferred minimum of one year experience in Pediatrics and/or Family medicine Recent Experience in the use of Electronic Medical Record documentation. Experience with Adult and Geriatric patient populations preferred Ability to work competently and efficiently with Microsoft Office: Outlook, Excel & Work Bachelor of Science Nursing (BSN) Licenses/Certifications Current licensure as a Registered Nurse in the State of California.
