The Clinical Manager is a dynamic and experienced licensed Registered Nurse, Physical Therapist, or Occupational Therapist, who serves as the cornerstone of clinical excellence within the agency. This individual is responsible for the comprehensive planning, coordination, and management of all multidisciplinary home health services, ensuring that patient care is not only compliant and effective but also delivered with compassion and integrity. Key Responsibilities 1. Conducts meticulous reviews and provides final approval for all clinical documentation, including OASIS data sets, plans of care (485s), and visit notes, to ensure accuracy, completeness, and regulatory compliance. Qualifications Education: Bachelor of Science in Nursing (BSN) from an accredited institution is strongly preferred. Licensure: Must hold a current, unrestricted license as a Registered Nurse (RN) in the state of California. Experience: A minimum of three (3) to five (5) years of progressive clinical experience as a Registered Nurse. A minimum of two (2) years of direct experience in a Medicare-certified home health setting is required. Previous supervisory or management experience in a healthcare setting is strongly preferred. Demonstrated experience with case management principles and practices. Certifications: Current CPR/BLS certification required. Oasis certification (COS-C or HCS-O) is highly desirable, or willingness to obtain within one year of hire.
