Overview Serving as a Home Health Aide CNA for hospice patients requires a uniquely compassionate person—someone who can manage daily care needs while providing a sense of comfort, stability, and normalcy within the patient’s home. Practically, this role means working as part of the Hospice Interdisciplinary Group under the supervision of a registered nurse or licensed practical nurse. Among our specialties: stroke, chest pain, minimally invasive and robotic surgery, cancer care and orthopedic and total joint surgery. Plus, a birth center and breast health center. Responsibilities Provides care delegated and supervised by a registered nurse to meet the needs of clients determined both initially and at two-week intervals. Participates in the development and implementation of the plan of care as delegated. Organizes and adapts plan of care with consideration for patient/client and family needs, desires and routines. Demonstrates awareness and reinforces priorities in the plan of care. Encourages self-care and independence as appropriate. Identifies normal and abnormal mental function and demonstrates skills for addressing the needs and behaviors of the cognitively impaired. Cares for and maintains client’s environment by avoiding environmental hazards, using restraints properly, and consistently using infection control procedures and barriers. Recognizes abnormal signs and symptoms of common disease conditions and communicates accurately and with timeliness. Provides and maintains basic nutrition and oral fluid intake. Recognizes emergencies and responds demonstrating knowledge of emergency procedures. Completes documentation according to guidelines. Qualifications Education: Completion of a Board approved nursing assistant training program. Licensure Current CNA and ability to meet state Home Health Aide Certification required. Current CPR certificate required. Valid driver’s license required. Pay Range USD $21.25 - USD $30.39 /Hr. Responsibilities Responsible for patient care within an assigned department/unit and participates as a member of the health care team in cooperation with and under the direction of the clinical manager or designee as defined by the relevant Standards and Scope of Practice for the Registered Nurse. The RN Staff Nurse assesses, analyzes, plans, implements and evaluates patient care following established patient care standards for Legacy Health System including those for patient assessment and transfer. Qualifications Education: As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration. Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized. Current acute care Endo experience in basic and advanced procedures strongly preferred. IV sedation experience preferred. Must be able to meet the 45-minute call requirement. Knowledge/Skills: Demonstrated ability of positive interpersonal and communication skills with colleagues, patients, families, physicians and the community. Keyboard skills and ability to navigate electronic systems applicable to job functions. Licensure Current applicable state RN license required. AHA BLS for Healthcare Providers.
