Facilitate communication between patients, providers, infusion services, imaging departments, specialty providers, and support teams. Collaborate with physicians and interdisciplinary teams to develop and execute individualized care plans. Assist with referrals, appointment coordination, treatment scheduling, and transitions of care. Monitor patient progress and proactively identify and address barriers to care. Provide emotional support and connect patients and families with appropriate community resources, financial assistance programs, and support services. Maintain accurate and timely documentation in EPIC and other clinical systems. Track patient outcomes and quality metrics to support evidence-based practice and program development. Serve as a clinical resource for patients, families, providers, and staff throughout the continuum of cancer care.
