Schedule: Part Time - Saturday and Sunday, 8am-4:30pm Summary: The Case Manager supports the interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. In addition, the Case Manager helps drive change by identifying areas where performance improvement in needed (e.g., day to day workflow, education, process improvements, patient satisfaction). Responsibilities: 1. Works collaboratively and maintains active communication with physicians, nursing and other members of the multi-disciplinary care team to effect timely, appropriate patient management. Addresses/resolves system problems impeding diagnostic or treatment progress. Proactively identifies and resolves delays and obstacles to discharge. Applies approved clinical appropriateness criteria to monitor appropriateness of admissions and continued stays, and documents findings based on Department standards. Ensures that all elements critical to the plan of care have been communicated to the patient/family and members of the healthcare team and are documented as necessary to assure continuity of care. Meets directly with patient/family to assess needs and develop an individualized continuing care plan in collaboration with physician. Documents relevant discharge planning information in the medical record according to Department standards. Collaborates and communicates with multidisciplinary team in all phases of discharge planning process, including initial patient assessment, planning, implementation, interdisciplinary collaboration, teaching and ongoing evaluation. Refers appropriate cases for social work intervention based on Department criteria. Initiates and facilitates referrals for home health care, hospice, medical equipment and supplies. Collects delay and other data for specific performance and/or outcome indicators, including but not limited to Avoidable Days, as determined by Director/Manager. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor, and resource to less experienced staff. Communicates relevant clinical information, including admission review, concurrent review, and discharge planning needs, to third party payors to secure optimal reimbursement. PURPOSE AND SCOPE: Functions as the hemodialysis team leader in the provision of chronic hemodialysis care and treatment. Provides day to day direction and supervision to assigned direct patient care staff. Communicates with the physician and other members of the healthcare team to interpret, adjust, and complete patient care plans. Provides safe, effective delivery of patient care in compliance with standards outlined in the facility procedure manual, as well as regulations set forth by the company, state, and federal agencies. Continuously monitor patient's condition with regards to problems and potential complications associated with dialysis. Administer medications to patients per physician's orders. Act as the subject matter expert and as a resource for staff members. Supervise and participate in completion of short- and long-term care plans. Admit new patients according to facility procedure. Ensure educational needs of patients are met and educate the patient and family about End Stage Renal Disease, dialysis therapy, diet and medication. Technical: Supervise the safe and effective use of all equipment involved in direct patient care. Operate all dialysis related and emergency equipment safely and efficiently when needed. Perform required testing and verification and initial the checklist for start-up and shut-down procedures as outlined in the Technical Services Manual. Complete Nurse's Technical Training Program/Water Quality Facility Training. Other: Assist with special projects or other duties as assigned by the Clinical Manager Assist with the interviewing of potential direct patient care staff as requested. Are you living your life's work? Why Join Our Team? Penn Medicine Princeton Health (PMPH) is seeking a full time Registered Nurse (RN) to work the night shift within the Transitional Care Unit (TCU). TCU and the Emergency Department (ED) are collaborating using a new split flow model. The split flow model allows emergency services to continue delivering care to arriving patients while the newly admitted patient is handed off to the TCU RN. The TCU is imbedded in and adjacent to the ED. Sign on Bonus $5,000 hiring bonus with 1 year commitment with Penn Medicine Princeton Health Scheduling Requirement: Three, 7p-7a shifts a week. 2-3 weekend night shifts required per 6 week schedule. Key Roles and Responsibilities: The TCU TEAM provides high quality, continuous nursing care to patients who have been admitted but remain in the ED while awaiting an inpatient bed. This role ensures safe, efficient, and compassionate care in a fast-paced environment, bridging emergency and inpatient services. May float to any med surg or rehab unit.
