In this vital role as a registered nurse, you will provide nursing support for respite patients throughout the BHCHP program. This position is crucial to assure that clinically appropriate and timely decisions are made, and actions are taken around patient assessments, administering of medications, treatments and overall patient care. Responsibilities: Direct nursing care of patients on assigned team to ensure that all needs of the patients are met, in conformance with respite established policies and procedures, as well as Department of Health rules and regulations. Responsible for accurate assessments and documentation, reporting to appropriate personnel as indicated (i.e., on-call provider, nurse manager, Director of Nursing, etc.). Responsible for administering medications and treatments to all the patients assigned to his/her Team skillfully and correctly, documenting per policy; observe and report any adverse reactions. Responsible for updating and maintaining accurate treatment records, flow sheets and vaccine administration, documenting appropriately. Participate actively in New Patients Rounds, Team Rounds, and all other Team meetings when appropriate. Responsible for giving and receiving report at change of shift; checking narcotics and related drug supplies per policy; sign appropriate forms. Responsible for taking off orders accurately, per policy; responsible for assigning specific individual tasks to, and directing Respite Aides, and giving each a verbal report on patients' needs. Maintains a professional approach with confidentiality; maintains the standards of accurate and complete recording and reporting. Ensuring that the clinical record for each patient on his/her Team is current and accurate. This position will provide comprehensive, high-quality primary, episodic and/or recuperative care at our main campus and surrounding shelter-based clinics. The ideal candidate for this role must be a self-starter and have the ability to lead a care team in partnership with other clinical peers. Our practice is collaborative, supportive and innovative. Responsibilities: Provides comprehensive, high quality episodic and/or recuperative care at various clinical sites staffed by Boston Health Care for the Homeless Program. May work at shelter-based clinic, hospital-based clinic, respite facility or on street-based outreach based on program needs. Elicit an appropriate history, perform physical examinations, order diagnostic studies, interpret results and initiate therapy in accordance with the Guidelines for Physician Assistants and Nurse Practitioners of Boston Health Care for the Homeless Program. Refer patients as appropriate to medical specialists, hospital admission, recuperative care, and detox facilities, providing appropriate information for continuity of care. Participate in collaborative relationship with BHCHP physician according to the Regulations of the Commonwealth of Massachusetts. Provide education to patients on health promotion and disease treatment and prevention. Work as part of multi-disciplinary team (medical, nursing, dental, BH, pharmacy, case management) in caring for patients. Administrative Responsibilities: Maintain adequate, up-to-date medical records, completed in a timely manner, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc. Maintain active DPH, DEA, and professional license, managed care credentialing and CPR training. Some on-call and holiday coverage required. Qualifications: Licensed by the Commonwealth of Massachusetts. Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency. At least one year of direct NP/PA experience. Life support training certification (e.g. CPR, BLS, ACLS) from a course that includes a hands-on, in-person component. If not certified prior to start date, applicant must submit certification within 90 days of start date. Position Summary The Inpatient Gastroenterology Consult Nurse Practitioner (NP) is an integral member of a busy inpatient gastroenterology consult service caring for children with a wide range of gastrointestinal, nutritional, feeding, and enteral access needs. Working closely with GI faculty, fellows, residents, medical students, nurses, dietitians, and other members of the multidisciplinary team, the NP evaluates and manages hospitalized patients across Boston Children’s Hospital. The GI consult service provides exposure to a broad and complex spectrum of pediatric gastroenterology, offering an opportunity to develop clinical expertise alongside experienced colleagues and trainees. The NP participates in daily rounds, develops and implements evidence-based care plans, coordinates diagnostic and therapeutic interventions, and supports safe transitions of care. Opportunities also include teaching, quality improvement, clinical program development, and scholarly activity.
