There are currently 4 consumers can admit 1 more with home health aide support. Bay Area Direct Client Care has an immediate opening for an experienced home health aide/CNA in the Apollo Beach area to care for a developmentally disabled female who is medically complex, under the care of a LPN. Shifts available are 7am-7pm and 7pm- 7am (NO SLEEPING IS PERMITTED). Days of the week are flexible to be full time or part time. The ideal candidate will have a minimum of 1 year experience. Although there is currently 1 patient, this is a group home and we can admit up to 5 patients (Differential Incentives for each additional patient admitted) Requirements: 1 year home health aide/CNA experience Current CPR/First Aide Highschool Diploma/GED Pass Local/Level 2 background screening Reliable Transportation 
Bay Area Direct Client Care has a contracted position for 1 patient in the family home who is medically complex. She requires medication via G tube and is incontinent of bowel and bladder with a history of seizures. The contracted dates are June 6,7,8,12, and 13th from 7am-7pm at a contracted rate of $40 per hour. Offer Based on Years of Experience What You Need to Know Be the Best Hospice Registered Nurse/RN Team Director You Can Be If you meet these qualifications, we would love to meet you: Associate degree preferred Certified Hospice Nurse (CHPN) preferred Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.) Minimum of one year of supervisory experience Possess and maintain valid CPR certification while employed in a clinical role (preferred) Must be a licensed driver who can travel to all business locations Meet the regulations and requirements of the state(s) in which the program provides services Responsibilities: As a Hospice Registered Nurse Team Director, you will : Supervise direct patient and family care team members, in collaboration with program site leadership Responsible for scheduling , payroll, and other administrative duties Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchonization Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdicisiplinary plan of care for each patient and family Collaborate with program site leadership memebrs to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols Pariticpates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines Collaborate with site leadership to support staff education efforts Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls Participates in or lead quality improvement activities, including data collection and aggregation as able Reinforces and supports all internal and external communications Our Investment in You Caring for others starts with caring for you. Shifts available are 7am- 7pm and 7pm-7am (NO SLEEPING PERMITTED). At this time days of the week are flexible and can be full time or part time. Note this is a group home there is currently one patient but we can admit up to 5 individuals (Rate increase for each new patient admitted). There is also home health aide present for personal care.
