This role also includes the delivery of direct skilled nursing care in accordance with physician orders. Working collaboratively with an interdisciplinary home health team, the Admission Nurse ensures high‑quality, patient‑centered care under the guidance and supervision of the Clinical Supervisor. Key responsibilities include evaluating patient needs, establishing care plans, coordinating services, and supporting continuity of care throughout the home health episode. This position follows a four‑day workweek , consisting of 10‑hour shifts , and includes a requirement to work one weekend per month and two holidays per year . Provide supervision and clinical guidance to patients and family members to promote independence and self‑care. Offer counseling to support patient understanding and participation in the care plan. Interdisciplinary Collaboration Work collaboratively with members of the multidisciplinary home health team to ensure coordinated, patient‑centered care. Communicate effectively with physicians, therapists, social workers, and supervisors regarding patient status and care needs. Clinical Documentation Complete accurate, timely OASIS documentation in compliance with regulatory and agency standards.
