As well, the Detox nurse works to educate the patient about his/her condition and to assist the physician and ARNP by the accurate clinical reporting of patient conditions, status and proper clinical actions. The Detox Nurse is involved in nursing assessments at intake and performs necessary discharge nursing actions as well as routine reports needed from clinical for insurance patients. This position is responsible for the proper administration, reconciliation, storage and security of all medications. The Detox Nurse reports to the Chief Nursing Officer. The specific tasks and responsibilities for this position include: Administers prescribed medications, as prescribed by medical team Makes independent decisions within the scope of their license involving patient care Uses sound judgment in the informing of the Chief Nursing Officer/Charge Nurse and the Medical Director of all decisions made and when/how they should be communicated Monitors and records through our Electronic Health System all aspects of the patients care which includes: Nursing assessments during intake Shift Assessments including CIWA and COWS assessments Changes in patient condition and updates on progress; other relevant documentation, that arise with each individual patient Discharge assessments and paperwork, follows Universal Infections Control Precautions using personal protective equipment as required, ensures that direct care staff performs these precautions as well as abides by Infection Control and Exposure policies Reconciles medication at the end of each shift which includes the responsibility of accurately maintaining logs of distribution of controlled substances in accordance to regulation Ensures that during shift change the nurse on duty is properly informed of any information needed to continue high standards of care for all patients. Full-Time | Rotating Shifts The American Association of Critical-Care Nurses (AACN) previously awarded the Children's National Pediatric Intensive Care Unit with a silver-level Beacon Award for Excellence. The three-year award marked an important milestone on the path to exceptional patient care and achieving a healthy work environment. The silver-level designation signifies continuous learning and effective systems to achieve optimal patient care and shows alignment practices with AACN's six Healthy Work Environment Standards. The Pediatric Intensive Care Unit (PICU) is a 46-bed unit. We will be expanding to 60 beds in 2027! This unit serves as a regional referral center and is a designated Level I Trauma Center for comprehensive care to pediatric patients and their families. The PICU Patient Care Management Team coordinates interdisciplinary patient assessment and patient care and provided by pediatric intensivists and specialty nurses in conjunction with multiple specialty services within the Medical Center. Patients are admitted to the PICU from the ED, Trauma Services, the Operating Room, and transfers from in-house units as well as transports from other institutions via the Pediatric Transport team. Trauma patients comprise about 10%-15% of patients in the PICU. These patients often have multiple injuries or severe head trauma. The PICU is also a Pediatric Burn Center and receives referrals from the region. Other complex patients requiring general surgery, ENT, craniofacial, neurosurgical and other surgical subspecialties are also cared for in the PICU. Medical patients comprise 50%-60% of patients and include children with respiratory failure, meningitis or seizures, sepsis, multi-system organ failure and chronically ill children. Children with cancer also require PICU care for complications that develop as part of the underlying disease, chemotherapy or following bone marrow transplantation. Services are provided daily, 24 hours per day. The Division of Critical Care Medicine (CCM) provides services and care to all patients admitted to the Pediatric Intensive Care Unit (PICU). Specifically, care is provided for all patients in the PICU, patients within the hospital by request, patients within the hospital as part of all emergencies and resuscitations, and patients in the ED for “medical alerts” and “trauma stats”. The scope of services includes resuscitation, life support, monitoring and treatment for life threatening events and diagnostic issues related to these items. Patients may be admitted to and discharged from the PICU based on pre-established guidelines. See section on critical care units. The department utilizes a multi-disciplinary approach to care, utilizing the services of physicians, nurse practitioners, nurses, respiratory therapists, social services and other support services necessary to provide quality care. Attending physicians providing care are board eligible or certified in Pediatric Critical Care. The PICU provides CAT (Rapid Response Services) and Code Blue team leadership to support in house clinical emergencies. An attending or fellow along with an ICU nurse and Respiratory Therapist respond to assist in clinical rescues of in-house patients. This service can also be activated by a concerned parent. All nursing practice is based on the legal scope of practice, national and specialty nursing standards, CNMC Policies and Procedures, and in accordance with all applicable laws and regulations. The Professional Model of Care requires registered professional nurses to be responsible and accountable for their own practice. CNMC supports the development of RN relationships within the community; specifically those relationships associated with the health and well being of the community at large. The Shift Coordinator, on a shift to shift basis, directs the activities of and/or participates as a member of the patient care delivery team. In collaboration with unit leadership, participates in planning, managing and evaluating unit based clinical care and ensuring that continuity of care is maintained. Salary Range: Minimum 39.64/hour - Maximum 61.45/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. Under general supervision, provides direct patient care to assigned patients. Education: Associate Degree from an accredited School of Nursing required. Licensures/Certifications: Current registration with the Maryland State Board of Examiners of Nurses as a Registered Nurse. Assessment. 1. Compiles database and formulates nursing diagnosis. 2. Utilizes physical assessment and interviewing techniques. 3. Anticipates need for intervention by other disciplines. 4. Determines priorities for patient care needs or problems. 5. Reassesses the patient as indicated by changes in condition and/ or response to treatment(s) B. Planning. 1. Initiates and updates a written nursing plan of care for patients by incorporating scientific and psychosocial principles that support the nursing diagnosis. 2. Incorporates information obtained from nursing care planning conferences into written nursing plan of care. 3. Coordinates patient, family, significant other, and health team members in the planning of hospital and post-discharge care. C. Implementation. 1. Performs clinical nursing skills independently. 2. Administers medications, IV therapy and treatments appropriately and accurately. 3. Initiates appropriate emergency procedures as necessary. 4. Appropriately delegates and coordinates the duties of ancillary personnel. 5. Communicates the patient's status and plan of care through written documentation in the patient's medical record. 6. Verbally communicates the patient's status to physicians, nurses and other health care professionals. 7. Communicates in a therapeutic manner with the patient, family and visitors. 8. Applies teaching/learning principles in education of patient and family. D. Patient Care Coordination 1. Manages daily patient assignment by appropriately setting priorities. 2. Delegates responsibility appropriately and monitors the nursing care delivered to patients by ancillary personnel. 3. Coordinates patient activities with other disciplines. E. Teaching and Quality Improvement 1. Develops, documents and implements Patient Teaching and Discharge Planning Record. 2. Participates in established patient, family, staff and/or student education program. 3. Participates in clinical monitoring at the unit level. 4. Identifies unit, staff and patient care problems. Purpose of Your Job Position The primary purpose of your position is to provide Nursing Care and to supervise the day-to-day nursing activities performed by Nurses, CNAs/GNAs and other nursing personnel. To monitor the performance of CNAs/GNAs, nursing and non-licensed personnel, provide education and counseling, perform disciplinary action as necessary, and complete performance evaluations. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our Center, and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times. Duties and Responsibilities Administrative Functions Direct the day-to-day functions of the Nurses, CNAs/GNAs or other nursing personnel in accordance with current rules, regulations, and guidelines that govern the long-term care Center. Ensure that all assigned nursing personnel including CNAs/GNAs comply with the written policies and procedures established by this Center. Periodically review the department’s policies, procedure manuals, job descriptions, etc. Make recommendations for revisions. Provide discipline and evaluations of assigned CNAs/GNAs and other nursing personnel in accordance with current rules, regulations, and guidelines that govern the Center. Issue verbal and written disciplinary warnings to assigned CNAs/GNAs and other nursing personnel for violations of current rules, regulations, and guidelines of the Center. Meet with your assigned nursing personnel, including CNAs/GNAs, as well as support personnel, in planning the shifts’ services, programs, and activities. Ensure that the Nursing Service Procedures Manual is current and reflects the day-to-day nursing procedures performed in this Center. Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual. Make written and oral reports/recommendations concerning the activities of your shift, as required. Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained. Ensure that all nursing service personnel are in compliance with their respective job descriptions. Participate in the maintenance and implementation of the Center’s quality assurance program for the nursing service department. Participate in Center surveys or inspections made by authorized government agencies, as may be requested. Periodically review the resident’s written discharge plan. Participate in the updating of the resident’s written discharge plan, as required. Assist in planning the nursing services portion of the resident’s discharge plan, as necessary. Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies, as required. Admit, transfer, and discharge residents, as required. Complete accident/incident reports, as necessary. Provide information to the Billing Office as necessary to insure accurate resident billing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. Education Must possess, at a minimum, a Nursing Degree from an accredited college or university, or graduate from an approved LPN/RN program. Specific Requirements Must possess a current, unencumbered, active license to practice as an RN or LPN in this state. Must be able to read, write, speak, and understand the English language. Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served. Must be a supportive team member, contribute to and be an example of teamwork and team concept. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities. Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel. Must be able to evaluate the performance, initiate disciplinary actions, and prepare and complete performance evaluations for personnel. Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. Must be able to relate information concerning a resident’s condition. Must not pose a direct threat to the health or safety of other individuals in the workplace. Must possess basic computer knowledge.
