Working with nursing staff and providers to deliver post-discharge care exclusively for trauma patients. Responsibilities include rooming patients in the clinic and assisting with incoming telephone calls related to AVS questions, home health, DME, prescriptions, referrals, scheduling, follow-up needs, along with other clinic tasks.
