JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 6. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 7. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills. 8. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. 9. Possesses and maintains CPR certification (may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer)

How you make a difference The Nurse Practitioner (NP) under the supervision of the Site Medical Director and other site Physicians, offers a comprehensive range of medical services to incarcerated patients in accordance with established policy and state guidelines. They ensure necessary documentation of services is provided to the Site Medical Director or designee to monitor the provision of clinical services. Additionally, they communicate scheduling changes to the Medical Director and H.S.A. and make arrangements for coverage of medical services if unavailable for an extended period of time. Key Responsibilities Conducts routine and emergency medical care for inmates, including chronic care clinics, infirmary rounds, and emergency treatment on-site. Adheres to established policies and procedures for medical care, including documentation in SOAP format, adherence to formulary regimens, and utilizing available in-house resources for treatment. Assumes responsibility and accountability for individual judgments and actions, while also reporting any concerns or questions to the Medical Director or Health Services Administrator. Maintains patient confidentiality and privacy in accordance with accepted practices for incarcerated individuals. Provides input into facility mandatory committees as requested. Additional Details This role will be a true PRN/As needed role, with coverage being needed periodically to cover time off for our full time providers. Candidates should be comfortable with substance withdrawal management, partnering with psych NP's and behavioral health staff, infectious disease management, MAT and standardized protocols (Title 15/NCCHC/ACA). In addition, candidates should feel comfortable doing suturing, incision and draining (abscesses are extremely common), basic wound care and debridement, splinting/minor fracture management, EKG interpretation, and emergency response (codes and overdoses) Qualifications & Requirements Education A graduate of an accredited school of nursing An advanced degree in an approved and accredited Nurse Practitioner program. Experience Prior medical/surgical and/or emergency/trauma experience and corrections experience is preferred. Must have the patience and tact to deal with the inmate patient population and ability to work effectively in the corrections environment. The NICU RNII is responsible for working in a team environment using a multidisciplinary approach to providing nursing care adapted to individual patient needs based on the nursing process, which includes assessment, planning implementation and evaluation. The individualized patient care is delivered in a safe, supportive, caring and family centered environment. Responsibilities The Clinical Manager of the Oncology Infusion Center has the responsibility, authority and accountability for patient care, administration and practice within the Infusion Center and for the development of the work force who consistently demonstrates competent, efficient clinical practice. Essential Functions and Responsibilities: Selects, schedules, assigns, and evaluates staff, adjusting hours and shifts as necessary to meet the needs of the unit and the patients. Coordinates orientation of new staff and assures that training and education needs of staff are met. Coordinates unit activities and the work assignments of staff members. Evaluates activities of department to ensure quality patient care, effective staff relations, and efficiency of service. Directs the nursing care of the Oncology Infusion Center. Observes nursing care and visits patients to ensure that nursing care is carried out as directed and care is delivered in accordance with physician instructions, regulations and standards of care. Assists in the development, implementation, and evaluation of unit policies, goals, and objectives. Participates in the development and modifications of unit programs and services. Participates in infection control, discharge planning, utilization review, case management, and quality assessment activities and analysis. Develops, interprets and enforces department and institution policies. Participates in the development and monitoring of unit operational budget and productivity standards. Participates in appropriate department and hospital committees. Directs preparation and maintenance of patients' clinical records. Inspects rooms and units for safety, cleanliness and comfort. Keeps informed and notifies staff of special orders/plans concerning patients or unusual cases. Investigates and resolves complaints and personnel issues of staff and/or patients. Collaborates with other departments and throughout hospital to provide needed services and/or care to the patient/family. Responsible for planning capital needs, ordering supplies, instruments, equipment, communicating with vendors and departments for the needs of the Oncology Infusion Performs other duties as assigned. Supervisory Responsibilities: Manages employees in the Oncology Infusion Center and is responsible for the overall direction, coordination and evaluation of this unit. Carries out supervisory responsibilities in accordance with the organization’s policies, and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Qualifications Required: Current CA Registered Nurse License Three (3) years of recent related experience Bachelors of Science in Nursing (BSN) Current Basic Life Support (BLS) certification Preferred: Masters of Science in Nursing (MSN) Current Advanced Cardiac Life Support (ACLS) certification CRNI (certified Registered Nurse Infusion) certification OCN (Oncology Certified Nurse) Member of ONS Knowledge of Det Norske Veritas (DNV) Overview Nationally Recognized.
