PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations. Non-Medical Health Support Helping Hearts provides non-medical personal support. Caregivers must remain within their authorized role and follow the client’s approved care plan. Depending on the assignment, responsibilities may include: Providing medication reminders Assisting with approved blood glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring the client’s physical, emotional and behavioral condition Reporting pain, swelling, bruising, redness or skin concerns Reporting falls, injuries or changes in mobility Reporting confusion or unusual behavior Following established emergency procedures Contacting the Helping Hearts office immediately when concerns arise Completing accurate electronic documentation Caregivers must report concerns rather than independently diagnose or treat them. Caregivers must not assume that someone else has already reported a change in condition. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Report any transportation concern immediately. Caregivers may only transport clients when the assignment has been approved and all transportation requirements have been satisfied. Night and Overnight Shift Expectations Caregivers working night or overnight assignments must remain fully awake, alert and attentive throughout the entire shift. Notify the office if a client or family asks them to arrive late, leave early or change the scheduled hours. Complete all required documentation before the end of the shift. Failure to report for an accepted shift, leaving a client without authorization or failing to follow safety procedures may result in removal from the assignment or additional corrective action. Adult ED Holds- The environment on our unit is friendly, focused on teamwork, and flexible to accommodate our patients’ needs. We care for adult patients 18 or older who are being treated for a variety of medical diagnoses. Our unit is unique because we have a blended emergency department and inpatient stepdown care delivery model. Our patient population encompasses a variety of diagnoses and levels of care. Shift This is a full-time night shift position. The shift is 7:00pm - 7:00am, three times per week for a total of 36 hours per week, including some weekends. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle.
