Job Summary: Within a team environment, provides support to providers and nurses during patient care encounters. Collects clinical information, completes point of care testing, ensures exam rooms are clean and stocked for patient visits, assists practitioners with examinations, treatment, or procedures, administers prescribed medications within scope, performs follow-up care per provider instructions, and schedules follow-up appointments as ordered. Resolves patient concerns promptly and maintains departmental quality Improvement standards. Essential Responsibilities: Adheres to Kaiser Permanente policies and procedures, principles of responsibilities, and applicable local, state, and federal laws. Utilizes effective communication and interpersonal skills when interacting with patients and members of the healthcare team. Completes patient intake, including vital signs, height, weight, and hearing and vision screening, as indicated. Identifies care gaps and schedules care gap visits/procedures, as indicated. Prepares consent, school, and work forms for provider to review and sign. Ensures exam rooms are clean and stocked with essential supplies and equipment for patient visits/procedures. Completes point of care testing. Administers in-scope immunizations and medications, as ordered, and provides patients with the most recent Vaccine Information Statement (VIS). Assists practitioners with examinations, including positioning and draping the patient, passing instruments in a sterile manner, and assisting with minor procedures. Documents patient care and education in the electronic medical record. Actively participates in Unit Based Team (UBT) and staff meetings, department-specific education, and quality improvement initiatives. May perform other in-scope duties as assigned.

JOB PURPOSE: Responsible for all duties in relation to safely administering medications and treatments to the residents. This position requires legible, accurate documentation on the medication record and in the resident chart. It also requires a responsible demeanor in order for the resident population to have confidence in your ability to administer medications and treatments. KEY RESPONSIBILITIES: Administers the medications and treatments, according to the scheduled times, following the medication administration protocol. Understands and appropriately documents on the 24 hour communication reports. Understands the resident alert status and documents appropriately in the residents record. Understands and implements an interim service plan, in the absence of Charge Nurse, Unit Manager/Director Health Services ,as care needs arise. Assures caregivers on shift are aware of changes on interim service plans. Notifies Charge Nurse,Unit Manager/Director Health Services of resident changes of condition. Assists with monthly medication records as needed. Assists with cycle fill or re-ordering of medications. Understands and returns medications for credit as needed. Monitors medications that arrive from the pharmacy. Timely notifies the pharmacy of any discrepancies Maintains the supplies, cleanliness, organization and re-stocking of the medication room and medication cart. Orders supplies as directed by the Charge Nurse. Has knowledge of, and can demonstrate proper technique, or can learn how to assist with: a gait belt, sit to stand lift, transfer board, catheter, specimen, transfer pole, trapeze, wheel chair, scale, and other equipment as directed by the Charge Nurse or Unit Manager. Capable of taking accurate vital signs and weights. Accurately begins a medication error report if necessary. Fills in as a Certified Nursing Assistant when needed. Capable of assisting in care needs of residents, which might include lifting requirements up to 40 pounds. Serves meals as needed and assists with setting tables and post meal clean-up as assigned. Faxes pharmacy to order medications. Calls pharmacy to follow up on medications that have not arrived. Answers call lights or finds a partner to answer a call light, if in the middle of a med pass. KNOWLEDGE, SKILLS, ABILITIES Assists residents with transportation and appointments as needed. Read the 24 hour communication book and respond as necessary to issues. Review all incident reports and residents on alert status Read any new interim care plans that could have been initiated. Provide the residents with their medications and treatments as per protocol Document medication administation in resident record Notify Charge Nurse, Unit Manager, DHS or Administator as needed when issues arise General record keeping and filing as needed. Evidence Based Practice and Research A. Promotes Evidence based practice, nursing research and performance improvement in nursing. Participates in activities such as value analysis as part of the decision making process in evaluating patient care products. B. Upholds all health care system/organizational policies and procedures and clinical competencies put forth by this job description and professional association including legal, regulatory and accreditation requirements and standards ensuring by way of example, such goals as TJC Patient Safety Goals and WellStar Health Systems safety absolutes. C. Participates in data collection, poses relevant clinical questions to advanced evidence based practice.
