Job Summary Summary The Patient Care Assistant II functions as a member of the patient care team, under the direction of the Registered Nurse (RN), and proficiently and consistently performs all aspects of the PCA role. The Patient Care Assistant II is responsible for supporting the RN with clinical tasks, including but not limited to blood glucose monitoring, completing EKGs, applying simple dressings, using aseptic technique and responding to medical emergency situations utilizing Heartsaver CPR and AED skills. Recognizes and notifies RN immediately of any significant change in a patient’s condition and/or monitoring alarms (for example: change in vital signs, change in patient’s mental status, complaints of pain, abnormal blood sugar, patient fall or other injury, patient concerns with care). • Accurately collects and records patient data (vital signs, intake and output, blood sugar, weights, restraint care/monitoring, other) and complies with hospital documentation standards to ensure a complete and accurate patient record. COLLABORATIVE INTERDISCIPLINARY • Maintains and fosters a teamwork approach to patient care. PATIENT/FAMILY EDUCATION • Addresses patient comfort and information needs by providing informative, nonclinical information to patients and families, orienting patients/visitors to the hospital, unit, patient room services, distributing patient mail/flowers, arranging room to ensure necessary personal care items are available & accessible (includes call lights), distributing nourishments, changing linens & clothing, refilling ice packs, running patient errands or other duties, as assigned by the RN. • In collaboration with the licensed nurse, reinforces the patient education provided by the nurse and provides patients/families with basic patient care instruction, such as instruction for using the call light, positioning, transferring, walking, toileting and feeding. COMPETENCY • Complies with established practice standards, policies, procedures, protocols, guidelines, and regulations, (for example: DPH, JCAHO, Boston fire codes, OSHA). • Maintains and demonstrates compliance with hospital, nursing and unit specific competencies and care standards. Maintains competence in relation to a variety of technical skills that may include, but are not limited to: application of external monitors, use of the CPM machine, performing EKGs and blood glucose monitoring, applying simple dressings and Heartsaver CPR and AED skills. • Assists in the orientation of new patient care assistants and serves as a resource to float staff. QUALITY IMPROVEMENT • Participates in hospital wide and unit based SEAQ programs and activities, as indicated. • Communicates any concerns related to quality patient care to the Nursing Director or Nurse Director, Clinical Leader and/or Nursing Supervisor. PATIENT CARE • Completes patient care, as assigned by the licensed nurse, according to hospital policy and procedure. Patient care includes: • Assisting with admission, discharge, and transfer of patients. Preparing room for admission and ensuring patients have appropriate identification bands upon admission and throughout their hospitalization. • Transporting patients and their belongings and their equipment and supplies, as needed and as part of the transfer/discharge process. • Providing personal care to patients including, bathing and skin care, shaving, mouth care, grooming, perineal care and changing linens, as per hospital protocols. • Providing basic skin care including but not limited to: back rubs, routine diabetic foot care, application of non-medicated ointments, application of elastic stockings, ace wraps, and compression boots, turning and repositioning patients every two hours, and other associated tasks. • Assisting the patient with elimination including, care of a urinary and condom catheter, use of bedpan, commode, toileting schedules, emptying colostomy bags, urinals and urinary drainage bags. • Carrying out planned patient care measures that provide patient comfort and pain relief such as, coordinating care to provide rest periods, minimizing noise, assisting in bed positioning. • Providing 1:1 monitoring as a patient observer, for patients at risk for injury and also providing the duties of the PCA II for the assigned patient. • Performing simple dressing changes, using aseptic technique. • Assisting nurse/health team members during simple and complex treatments or procedures. • Setting up patients for meals and assisting them with feeding and opening food containers, maintaining calorie counts and recording intake/output measures (I and Os). • Taking patient vital signs, completing intake and output data and daily weights accurately and documenting results completely. • Performing EKGs as directed and according to Hospital and Nursing Department Policy and Procedure. • Performing blood glucose monitoring as directed and according to Hospital and Nursing Department Policy and Procedure. • Documenting patient information in a timely, accurate and complete manner, on patient flow sheets. • Providing end of life care, including comfort measures only and post mortem care. • Collecting, labeling, and sending/transporting specimens to the lab with the appropriate lab slips/requisitions attached. • Initiating patient belongings tracking form and properly securing patient belongings in accordance with hospital policy and in conjunction with the RN. Consults with RN, if patient declines to secure their belongings or have their belongings sent home with family member or friend. • Responding promptly and courteously to all patient call lights and patient requests and seeking assistance from co-workers, as appropriate. • Maintaining patient rooms in an orderly and clean manner and stocking patient rooms, with needed supplies and equipment. • Making routine room rounds to un-clutter, clean and organize patient rooms/areas, when needed. • Checking inventories and restocking/replacing/assembling patient equipment and supplies. • Disposing of trash, soiled linens, medical waste and disposable items, appropriately. • Initiating and performing spot and spill cleaning, as needed. • Performing regular checks per policy and procedure to ensure that equipment is operating properly and has current safety inspections/expiration dates, before patient use. • Identifying broken/expired equipment/supplies, removing from service and notifying Charge RN/Staff Nurse. • Performing initial cleaning/decontamination of equipment/materials, e.g., removing reusable equipment from isolation rooms, and arranging for terminal cleaning, as appropriate. • Identifying emergency/crisis situations and initiating appropriate response actions, within the scope of her/his role. Responses include immediate communication of such events to the RN and assisting the nurse during and after the emergency, as directed. PATIENT CARE • Completes patient care, as assigned by the licensed nurse, according to hospital policy and procedure. Job Summary Summary Under the supervision of the Nursing Director and as a member of the Unit Based Patient Care Team, the unit secretary performs a wide range of secretarial duties and coordinates unit/floor procedure to ensure professional and efficient operation of the department in providing quality patient care. Performs the following duties to support unit operations and coordination of resources including but not limited to, answering unit phones, collaborating with health care team members and other departments, scheduling patient tests and appointments, maintaining all medical records, initiating and tracking unit requests, coordinating discharges, transfers, and admissions proactively to assure efficient operation and management of resources on the unit. Demonstrate the knowledge and skills necessary to provide indirect patient family centered care appropriate to the age of the patients served on assigned unit. No Essential Functions Provision of a Safe Environment - Assists in maintaining a safe environment and intervenes appropriately. - Identifies and/or initiates appropriate follow up for any safety issue. - Utilizes judgment in implementing physician orders in a safe and conscientious manner, notifying charge nurse immediately of any concerns. -Service - Ensures patient and family member’s needs and expectations are met in a timely and caring manner. - Maintains the C.A. R. E. standards with focus on communication, appearance and respect. - Works collaboratively as a member of the Patient Care Team and takes a proactive role in ensuring the smooth and efficient operation of the unit. -Organizational Ethics - Maintains and promotes the mission and values of the organization. - Maintains conf identiality of all patient information - Respects and supports a culturally diverse staff and patient population. - Utilizes the proper procedures address patient or family’s concerns by notifying the Nursing Director/supervisor or clinical leader. -Management of Resources/supplies and unit based needs - Orders, expedites and distributes unit, clerical and patient care supplies as directed by the Nursing Director to assure par levels required of efficient operations. - Orders supplies (including Workflow One) necessary for the unit as described by the Nursing Director through eBUY. - Maintains work area and office equipment in neat and functioning order. - Initiates and tracks appropriate Facilities, Bio Med, Housekeeping, and IT related problems/replacements/ requests or work orders. - Notifies Biomed department of malfunctioning equipment and arranges repair requests and for the equipment to be returned to the department. - Assists Nursing Director to manage and facilitate unit projects. - Acts as a liaison and assists in problem solving with ancillary services and all other departments which provide services or have an effect on unit activities. - Provides back-up coverage for patient transport, including escorting discharge patients/families. -Management of Information - Answers, coordinates and manages all phone calls to the unit in consultation with the charge nurse. Takes accurate messages and ensures their delivery to the appropriate staff in a timely and professional manner. Places pages and phone calls upon request of clinical staff. - Participates in shift-to-shift hand off communication by giving or receiving hand of report to/from the incoming/outgoing unit secretary and charge RN. - Coordinates and notifies appropriate staff regarding all paperwork coming onto the unit including fax referrals. - Maintains all patient medical records per hospital policies and procedures in a neat and orderly manner; properly label each page and file testing results and other information in individual medical records accurately and in a timely manner. - Schedules patient diagnostic tests, appointments, and consults upon the direction of physician or RN. - Coordinates logistics of patient discharge including transportation, equipment, etc. - Performs clerical duties to admit, transfer, and discharge patients with the use of bed management system. Make certain that the patient record/chart, information plate and/or labels, personal belongings, and other pertinent information accompany the patient on transfers. - Enters, updates or prints data reports as appropriate including: -Bed management system in a timely manner to reflect current unit activity. -Diet order sheets ready for pick-up by food and nutrition services. -Advance Care Directives and Patient Code Status in the chart each shift. -Medication Reconciliation and Restraint reports and provides them to the physician or RN as a reminder to reorder restraints or update/complete medication reconciliation each shift. -Quadramed assignment sheet and provides to the charge nurse prior to the beginning of each shift. -Recovery charges, and or billing information as indicated - Checks the quadramed list by 11:30am and notifies the charge nurse of any unclassified in-patients as well as reminding staff to complete their quadrameds. - Copies patient charts and other materials as requested. - Utilizes Meditech system to: access and update administrative data screens, to enter transport requests, enter PASSPORT orders, print specimen labels, print kardexes, print medication administration report. - Requests and maintains old medical records as needed. - Anticipates discharges and transfers and calls them to admitting/nursing supervisor when discharges are known and inputs any tentative discharges in bed management system. - Updates charge nurse/clinical leader, admitting and nursing supervisor of admissions, discharges, and transfers on 7-3 and 3pm-11pm shifts and inputs any tentative discharges in bed management system. - Communicates with healthcare team members via VOCERA communication device. - Performs all other duties as assigned. -Collaborative Interdisciplinary Care - Interacts with all members of health care team to provide interdisciplinary patient family centered care. - Answers patient call lights and communicates patient request to appropriate clinical staff. - Provides clerical support to unit staff as needed. - Floats to other pods or units as requested by Nursing office and functions appropriately as reported by staff. -Patient/Family Education - Receives and orients visitors to unit/department in a professional manner, projecting a positive public image with use of the C.A.R.E. standards. - Provides all patients and families with basic information for unit operation. -Competency - Maintains compliance with hospital, unit secretary specific competencies. - Identifies and actively seeks out unit and/or service related continuing education classes. - Orients or precepts new personnel as assigned and acts as a resource for other professionals, patients, and significant others on the unit. - Responds to staffing needs of the healthcare team by cross-training and floating to other units. - Attends staff meetings and/or read staff meeting minutes to keep current with unit and organizational updates or changes. - Attends the annual Safety Fair. -Quality Improvement - Understands unit specific quality indicators. - Participates in Quality Improvement Projects and Committees as directed by the Nursing Director. - Participates in Service, Excellence, and Quality (SEAQ) programs for example, Falls Team, Patient Safety Committee, Pain Management Team -Patient Care - Responds to patient and family care by answering patient call lights and telephone in a timely manner. - Coordinates patient communication through a variety of general reception and clerical duties Grade 130 Qualifications Education High School Diploma or Equivalent required Can this role accept experience in lieu of a degree?
