Schedule/Shift : M-F 8-5pm Territory/Location: Mission Home Health of Carson City - Reno, NV Responsibilities: Oversee day-to-day office and clinical operations, ensuring continuity of services, completion of daily and weekly tasks, and appropriate staff coverage during absences. Supervise clinical and administrative staff, including RN and PT Case Managers; monitor action screens, schedules, productivity, and overall workflow. Develop and manage on-call schedules, review on-call logs, provide administrative on-call support, and ensure smooth off-hours operations. Review, approve, and follow up on clinical documentation including plans of care, evaluations, OASIS, 485s, chart audits, complaints, and quality improvement reports; address documentation deficiencies as needed. Ensure individualized, appropriate plans of care are executed as written and reassessed per regulatory and physician requirements; ensure proper documentation for transfers and discharges. Manage intake, referral, non-admission, internal transfer, admission, and billing processes, including non-Medicare referrals, to ensure timely reimbursement and minimize outstanding claims. Approve schedules, payroll transmittals, non-visit activity, and accounts payable in accordance with company protocols and timelines. Complete and distribute required audits and reports; address internal audit findings and maintain required clinical, operational, and HR performance benchmarks. Ensure compliance with state, federal, and internal regulations; maintain survey readiness and support all external audits. Lead and participate in staff meetings, aide in-services, case conferences, and ongoing education initiatives. Support field operations as needed, including patient visits and participation in on-call rotation when applicable. Participate in community outreach, disaster preparedness planning, drills, and emergency response; serve as Disaster Coordinator in the absence of the Administrator. Remain available during operational hours and perform additional duties as assigned by the Branch Director. Qualifications: Graduate of an accredited school of nursing. Bachelor's degree preferred, with a minimum of 3+ years of home health care experience. Must have a minimum of 2+ years of nursing experience within the last 36 months. Medicare home health experience required. Current licensure in State, CPR certification Must have a good understanding of the Federal, State and local laws/regulatory guidelines governing the operation of a Home Health Mission Healthcare. Have valid driver’s license with no restrictions and auto insurance meeting state’s insurance coverage minimums. Responsible for supervising home health aides. Supervisory experience not required but preferred. 2 years of Home Health experience required. 3+ years of Home Health experience preferred. Preceptor experience preferred. US:NV:Carson City Emergency Department Full Time * Schedule expectations : After initial 90 days, majority of schedule would be as an Educator with a secondary assignment as Charge RN and Floor RN worked 2-4 times per pay period. Assists personnel in the admission, perioperative care and discharge of patients, specializing in gastroenterology. Utilizes the nursing process to develop a plan of nursing care. The RN assumes responsibility for assessing, planning, implementing, evaluating direct and indirect nursing care, identifying outcomes for patients in the Perioperative setting. Summary of Position: The Nurse Practitioner (NP) - Pediatric position provides direct patient care within state and scope of practice guidelines at assigned Barton HealthCare System outpatient clinic facilities. We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges. Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs. Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication. Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members. Educate patients and families on disease progression, symptom management, medications, and hospice philosophy. Communicate significant changes in patient condition promptly to hospice and attending physicians. Maintain accurate, timely clinical documentation including assessments, care plans, and notes. Assist with rescheduled, missed, or reassigned visit requests based on triage direction. Participate in calendar and scheduling modifications, including service codes and clinician assignments. Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection. Assist with TPE reviews, QAPI efforts, and regulatory compliance. Ensure infection control standards and universal precautions are consistently followed. Provide leadership and guidance to field staff; conduct supervisory visits when appropriate. Participate in required meetings, after-hours updates, and trainings. Maintain proficiency in pain and symptom management for hospice and end-of-life care. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. This position performs a dual function, clinical and managerial. Provides direct and indirect patient care in the clinical setting, participates in hiring of staff. Communicates with physicians/Nurse Manager/Director/co-workers, as appropriate about changes in patient’s clinical condition including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Job Duties/Responsibilities: Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care. Ability to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes pediatric, adolescent and geriatric patients and the general patient population. Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Uses triage process to ensure timely and appropriate care to patients. Accurately assigns triage categories.
