Accurately communicates/documents medication doses prescribed, and provides patient education on medication administered. Follows up on patient care responsibilities; communicates incomplete assignments and patient needs with other disciplines to assure continuity of care; transcribes orders accurately and performs chart audits of transcription accurately and according to facility guidelines. Utilizes nursing knowledge to successfully meet the needs of the facility, patient population and staff members. Utilizes nursing resources to enhance evidence-based practice. Guides, educates, and provides leadership as a preceptor to new staff. Performs other duties and projects as assigned.

Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC). Qualifications Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed professional. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Experience as a licensed professional may be deferred with a review and approval from regional leadership. Medicare home health or hospice experience is preferred.
