Responsibilities PRIMARY Ability to provide quality care in both clinic and telehealth and meet patient volume goals targeted for tele and in patient as determined. Ability to meet patient volume goals targeted for tele and in-patient as determined. Ensure accurate completion of patient charts in a timely matter and forwarding charts as appropriate on a daily basis. As well as build and maintain confidence and credibility with all employees. Implement clinical and Telehealth protocols as outlined CMO and Supervising Physicians. Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses. Deliver excellent patient care through in-patient and virtual diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information. Ensure that the activities of the Provider are conducted in a manner that is consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues such as HIPAA. Ability to work efficiently in a fast-paced, autonomous environment within both in-patient clinic and virtual settings. Ability to pursue queue of telehealth patients in a timely fashion to ensure proper patient follow up. Order, perform, or interpret the results of diagnostic tests, as well as responsibly prescribe medications and educate patients on continued treatment and care of acute and chronic conditions. Attest and follow clinical practice guidelines by the Office of Medical Affairs. Attend Mandatory monthly meetings with Supervising physicians and E/M training. Ability to perform responsibilities within standard NP/PA protocols. Responsible for learning the aspects of compliance in the company by completing all mandatory compliance training in order to meet and exceed our continued quality of care. Current License or Certification: License and DEA must be active, in good standing, and verifiable with the proper regulatory agency. DEA required for all providers; however, where limited by years of practice under state law, DEA will be required within 120 days of provider meeting the minimum years of practice under state law for obtaining DEA. Providers must be able to treat all ages and must meet any credentialing requirements needed; and DOT certification is required to be obtained by FT and PT Providers within 120 days of employment and maintained during employment. What You’ll Do As a Pediatric Home Care Specialist & Acute Care Registered Nurse, you serve as the clinical bridge between home‑based care and virtual acute care. You will deliver high‑quality, family‑centered care in the home and community while flexing into virtual acute care nursing to support urgent pediatric needs. In this role you will: Travel to members’ homes and community settings to deliver episodic, longitudinal, and urgent pediatric care. Build trusted, compassionate relationships with children and families, ensuring coordinated, culturally responsive care. Collaborate within an integrated care team including physicians, APPs, nurses, pharmacists, dietitians, navigators, social workers, and therapists. Conduct and document in‑home assessments; perform procedures such as ear irrigation, point‑of‑care testing, IV placement and lab draws, wound care, and medication administration. Provide patient and family education, follow‑up care, and care coordination using the Imagine Pediatrics platform. Flex into the virtual acute care RN role to conduct virtual assessments and manage urgent pediatric needs using established protocols. Document care in the EMR (Athena) and communicate with families through Imagine Pediatrics’ proprietary application. Utilize virtual platforms and digital tools confidently to support care delivery. Active TN RN license required, and compact license preferred. Current residence in or around Nashville area PALS & BLS required Bachelor's degree in nursing (BSN) is required. EMS (EMT/P) experience and 2+ years of pediatric home care experience preferred Bilingual Spanish is preferred Minimum 3+ years of pediatric nursing experience, ideally including high‑acuity settings (ED, urgent care, transport, or similar). 1–2 years in pediatric emergency medicine experience. Valid driver's license, reliable transportation, and good driving record. Comfort driving long distances in varied weather conditions. As a 1099 Independent Contractor, you’ll have the flexibility to set your own schedule while providing routine healthcare services in clients’ homes. This role allows you to design a work schedule that suits both your needs and those of your clients. Responsibilities: You will follow care plans to provide safe and competent care, assist with personal hygiene and meal preparation, help with mobility, and perform routine housekeeping tasks. Additionally, you will monitor and report any changes in clients’ conditions or home situations to the RN Case Manager and HHA Coordinator. Qualifications: High school diploma or equivalent. Completion of state-mandated training hours, if applicable. Basic knowledge of computer and internet-based applications for charting. Strong problem-solving and multitasking skills. Compassionate and professional demeanor, with safe working knowledge of client-based equipment. Must pass a criminal background check and provide evidence of health status that does not pose a risk to clients or other caregivers.
