This position works in collaboration with physicians to provide complete physical examinations, diagnosis, and treatment of both chronic and episodic disorders. Assigned principal responsibility for primary health care of patients. Seeks the expert opinion or advice of a physician whenever the incumbent decides a case falls outside the scope authorized by policies and protocol. The Nurse Practitioner typically remains responsible for the patient throughout the course of treatment, provides the physical examination, the diagnosis, and the health care plan, determines when the patient has recovered, and releases the patient. The Nurse Practitioner may refer the patient to a physician for consultation or to specialized health resources for treatment, but retains responsibility for clinical management as well as any subsequent modification. Responsibilities Assesses the physical condition of a group of patients by performing physical examinations including ordering, interpreting and evaluating diagnostic tests and obtaining medical histories. Assumes ongoing responsibility for each patient from discovery of the disorder to recovery. Formulates the medical plan and prognosis based on patient's condition. Consults with physician(s) regarding patient health care plans and treatment. Orders, interprets, and evaluates diagnostic tests to identify and assess the patient's clinical problems and health care needs. Performs selected invasive procedures for therapeutic and/or diagnostic reasons within scope of validated competence. Initiates and/or recommends medical drug therapies or other forms of related therapeutic treatment and procedures in accordance with approved protocols/policy. Complies with medical records policies regarding the complete and accurate documentation of patient care. Initiates and maintains required records and legal documents. Participates in quality improvement activities. Counsels patients, families and significant others about health and illness, and promotes health maintenance. Establishes and documents the health care plan and prognosis, maintaining records of each case and evaluates the effectiveness of the plan. Initiates drug therapy, taking full responsibility for such therapy. Consults with a physician prior to implementing the therapy, as necessary. Determines when the patient has recovered from the disorder and releases the patient. Qualifications Education: As required by licensure. Experience: Clinical experience requirements are dependent upon the specific position, and include a minimum of 2 years of experience in a related field. Skills/Knowledge: Basic familiarity with computers preferred. Licensure Current applicable state RN license required. National Certification as a Nurse Practitioner (NP) required. Current applicable state license as a Nurse Practitioner with Prescriptive Privileges (NP-PP) required. Overview LPNs take care of patients that have long term treatment needs. Often, these patients do not have a safe discharge plan to complete the treatment elsewhere and therefore continue to have a bed in the hospital. LPNs could have the opportunity to have hands on experience with wound care and IV antibiotics, among other long term care treatment. LPNs may often work beside acute care RNs but not directly in acute patient care areas. They work in collaboration with a C.N.A., charge RN, unit coordinator and physician. LPN staff with a variety of experience and backgrounds work on this unit. We strive to support learning and continuted education with all staff. Complex discharge units have medically stable but complex patients who are pending a safe discharge plan. The LPN must think critically, be able to problem solve and be ready to learn. Excellent communication with teammates and patients is essential. The ability to maintain compassion for this complex patient population is also essential. Staff on this unit value teamwork. Our physician partners work closely with this team to support in providing the best care posible for our patients. Responsibilities Assessment : Contributes to and communicates age appropriate admission and daily bio/psycho/social assessments. Collects, reports and records objective and subjective data in an accurate and timely manner. Observes the patient's condition or changes in condition. Documents the observations in a timely, legible and concise manner in accordance with written guidelines. Validates patient classification indicator via required documentation. Analysis: Contributes to the establishment of nursing diagnosis and clinical problem identification. Identifies and reports the significant deviations from the norm to the RN, MD and oncoming shift. Planning: Participates in establishing and updating and individualized plan of care in collaboration with RN, patient and family. Provides data and contributes to the identification of priorities. Involves the patient/family in individualizing/modifying the nursing plan of care in collaboration with the RN. Reviews and updates the nursing plan of care to reflect current patient status by the end of the shift. Implementation: Participates in implementing the planned nursing and medical interventions with knowledge and competence in accordance with written standards of practice. Provides care for patients whose conditions are stable or predictable under minimal supervision of the RN or MD. Cares for patients whose conditions are critical and/or fluctuating under direct supervision of the RN or MD. Carries out the planned interventions with knowledge and competence. Uses interpersonal skills that convey a positive and supportive attitude. Determines and communicates patient/family desired level of participation in care. The TRACU (Trauma Recovery Acute Care Unit) is a 33-bed unit that provides care to adult trauma patients. This unit provides care for the patient with acute neurosurgical, orthopedic, and other multisystem trauma injuries. Priority for admission is given to any adult patient with neurosurgical or orthopedic trauma. A variety of interventions including portable ventilators, multisystem trauma care, pain management and blood/blood products administration pre/post op care of adult neurosurgical patients including those requiring ventriculostomy management, may occur in this patient population. Dedicated Urgent Care Specialty – exclusively practice urgent care medicine – No Primary Care, DOT or Occupational Medicine in our urgent care position . Qualifications NP: 1-2 years’ experience in an Urgent Care or Emergency Medicine setting required. Experience treating patients ages 6 months and up in an urgent care or emergency room setting. Ability to comfortably perform minor procedures (sutures, casts, splints, etc.), overview reads of EKGs and X-rays. Licensure Current applicable state RN license required. National certification as a Nurse Practitioner (NP) required. Applicable state license as a Nurse Practitioner w/Prescriptive Privileges (NP-PP) required. Current certification dependent upon the specific position to include Basic Life Support (BLS) from the American Heart Association. Pay Range USD $0.00 - USD $0.00 /Hr.
