Assistant Healthcare Coordinator Needed in the Bronx. Full time position! We are seeking an individual that enjoys being creative and is passionate about working with vulnerable populations. This role will allow you to design trainings as well as education models for tenant populations. The goal of these trainings will be to reduce unnecessary in-patient hospitalizations as well as emergency room use, provide support to on-site staff on best practice for a variety of healthcare needs, including mental health, and to provide the tenant with the opportunity to learn how to independently manage their healthcare needs. Responsibilities: Develop, and monitor healthcare related metrics within 4 sites. Review in-patient and ED discharge instructions to create a follow-up care plan for the tenant. Collaborate with Case Managers to create individual healthcare service plans for tenants, including medical and mental health needs. Coordinate Vaccination and Flu Clinics. Assist in planning and executing quarterly Healthy Events. Design a program to monitor tenant Health Insurance status. Create group trainings for Case Conferences/Team Meetings regarding best practice for both medical and mental health issues. Consult with staff regarding best practice for substance abuse disorders. Provide education on trauma informed care as well as harm reduction. Consult with individual tenants as well as staff regarding healthcare concerns. Assist with pillboxes and vital signs as needed. Develop collaborative relationships with healthcare settings to improve the referral process and patient care communications. Requirements: Licensed as an LPN in the state of New York. Bilingual Spanish/English required. 1-2 years of experience. Willingness to work at multiple sites. Experience with LGBTQ populations is preferred. Candidate must possess superior written and verbal skills. Candidate must be well-organized and possess multi-tasking skills. Candidate must have a knowledge of medical terminology. OVERVIEW: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and the Nursing Policies and Procedures. As an LPN you will be responsible for providing residents assistance with ADLs and in the administration of their medications while overseeing staff. Adhering to the instructions of Nurse Management, you will perform your duties in accordance with all company policies and procedures, while providing resident care services with kindness and compassion to the residents. RESPONSIBILITIES: Holds daily shift report with nursing assistants assigned to the unit prior to the start of the shift. Reviews written daily assignments for each resident on assigned unit. Assures assignments are completed before the end of the shift Makes rounds on assigned residents at least once every two hours Administers medication and treatments according to the physician's orders. Assumes responsibility for ordering medications Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a monthly basis or more often if the resident's condition changes Assists the attending physician with rounds and special procedures as needed Transcribes, documents and implements physician's orders Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering Assumes independent responsibility for yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps taken to improve their performance. Such evaluations become part of the nursing assistants’ permanent records and shall be used as the basis to determine the continued employment of nursing assistants, promotional opportunities and the amount of any wage increases granted Assumes independent responsibility for documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and recommending the suspension or discharge of an employee Assumes independent responsibility for completing required routine documentation and auditing as required Participates in the hiring, interviewing and orientation of nursing assistants Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: Practical Nurse licensed in the State of employment Minimum one year of education or experience in geriatric nursing or rehabilitation preferred Knowledge of nursing theory and practice, including the administration of medications Ability to read, write and comprehend English CPR certified SUPERVISORY RESPONSIBILITIES: This position has no supervisory responsibilities.
