Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Job Description The Department of Pediatrics at SUNY Downstate Health Sciences University is seeking a full-time TH Assistant Director of Nursing / Nurse Practitioner, Outpatient Gastroenterology Infusion Center. The successful candidate will: Provide advanced care, including administering intravenous medications, fluids and nutrients to children to treat chronic illnesses, infections, or for wellness purposes like dehydration and fatigue. Demonstrate responsibility for patient management assessing and educating patients and families, and monitoring patients before, during and after therapy. Oversee pediatric infusions, providing a higher level of care and diagnostic capabilities, including ordering necessary lab tests and adjusting treatment plans. Work both independently and under physician supervision, to create treatment plans, start infusions, monitor for reactions, and educate families. Perform patient assessments, manage medications, order diagnostics, and assist with procedures like endoscopies and colonoscopies. Provide assistance in coordinating, scheduling of the outpatient procedures and prior authorizations for infusions. Coordinate patient care for infusions and therapy. Document all care provided in the patient's medical record; completes all required patient care documentation and submit all documents to the office. Develop further and manage patient care panel with participation in managed care programs, and other direct patient care programs as applicable. Participate in and provide departmental education activities to medical students, residents, fellows and allied health professional students. Participate in hospital and departmental health fairs, community education events and speak to community groups when requested. Illustrative Duties Graduate Nurse (T&RC) Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Carries out treatment procedures, administers medications, and monitors individuals’ conditions. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Participates in the development, implementation, and revision of individual care plans as a member of a multidisciplinary treatment team. Modifies nursing care plans based on individual condition and responses to care and treatment. Triages individuals by inquiring about illnesses, injuries, and symptoms, and records findings and observations. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert.
