Reviews diagnostic test results and reports abnormal results and changes in patient condition to the Registered Nurse, physician and appropriate healthcare team members in a timely manner. Collaborates with the Registered Nurse on development and implementation of the patients plan of care. Administers prescribed medications, therapies and treatments in accordance with Licensed Practical Nursing Standards. Maintains and reviews patient medical records, notes, and physician orders. Documents all patient care activities in accordance with documentation standards. Assists physicians, Registered Nurse and multidisciplinary team with data collection, planning, intervention and evaluation of patient and family care to achieve desired outcomes. Implements components of the plan of care with the Licensed Practical Nurse scope of practice and competency to promote desired outcomes. Assists with discharge planning and carrying out the patients teaching plan under the direction of a Registered Nurse. Evaluates and maintains the environment to promote patient and healthcare team safety. Collaborates with the Registered Nurse and multidisciplinary team to advocate for the patient and supports the patient's psychological, ethical, spiritual and cultural needs. Provides compassionate basic nursing care; maintains patient privacy and confidentiality. Communicates effectively and works collaboratively within the interdisciplinary team. Job Summary and Qualifications As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a key role in delivering safe, high-quality care that puts patients first. Home Health Licensed Practical Nurse LPN

The LPN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required. ESSENTIAL DUTIES AND RESPONSIBILITIES The essential functions include, but are not limited to the following: Provide skilled nursing care under the direct supervision of the Clinical Supervisor Complete evaluation tasks, including vital signs and medication reviews Coordinate with physicians, therapists and other individuals in the client’s care plan Assess and chart observations of the client’s condition at each visit Maintain complete and accurate documentation of nursing care Work collaboratively with all members of the health care team Provide education to clients and families on proper home health care procedures and strategies Record patients' medical information and vital signs. Monitor, record, and report symptoms or changes in patients' conditions. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans Modify patient treatment plans as indicated by patients' responses and conditions Monitor all aspects of patient care, including diet and physical activity Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems Perform other duties as assigned MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) Associates degree required; Graduate of an accredited state approved, Bachelor’s Degree from school of nursing preferred Current and valid Virginia Licensed Practical Nurse license One year of LPN experience Current CPR certification (required) , Driver’s License , Car Insurance , Car Registration Strong interpersonal skills that build personal rapport and establish trust with the client Computer literate Strong organizational skills Excellent written and communication skills Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers) Home health care experience Bilingual (Spanish) preferred PHYSICAL DEMANDS AND WORK ENVIRONMENT The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
