As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. Your Role & Responsibilities You will provide comprehensive care to seniors following clinical standards. You will collaborate daily with your care team to ensure coordinated, high-quality care. You will manage referrals, hospital/SNF coordination, DME, and home health services. You will document care accurately and efficiently with support from quality-based coders. You will participate in on-call rotation and support growth plans at your center. You will have passion for providing value-based care. What We're Looking For Required: You will have a Master's Degree in Nursing or completion of a PA program with board certification. You will have an active, unrestricted NP or PA license in the state of practice. If selected for this role, you will be required to be screened for TB Preferred: You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings. You will have an active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers #LI-KB2 Use your skills to make an impact Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $123,800 - $170,300 per year This job is eligible for a bonus incentive plan. As the Home Health Patient Care Manager, you are responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations. Primary Responsibilities: Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team Receives referrals, ensures appropriate clinician assignments, evaluate patient orders, and plot start of care visits Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals Ensures patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
