Description The Hospice Access Field and Triage RN will complete medical requirements while coordinating a whole-person plan of care. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. In Field role, the RN will visit patients in their homes (private or facility) to present the hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and interdisciplinary team (IDT). Field RNs will also make urgent and routine home visits to patients already on hospice service. In Triage role, RNs will take patient calls from Halsey Building in NE Portland. Department Overview This position will support in-clinic both primary care clinics located at Beaverton Clinic and Orenco Station. In addition, this position will support additional primary care clinics within the Family Medicine and Internal Medicine Departments (remotely) if Beaverton and Orenco clinics do not have staffing needs. Function/Duties of Position Primary Care at Orenco and Beaverton clinic serves both Family and Internal Medicine providing comprehensive primary care to patients across the lifespan, including pregnant individuals, infants, children, adolescents, adults, and older adults. Patients are often moderately stable to moderately unstable, with complex medical conditions and significant social determinants of health (SDOH) needs. A portion of nursing work occurs outside of scheduled provider visits. Nursing services include RN visits, telephone triage, obstetric intakes and support, medication administration, patient education, and transitions of care and RN care management. RN visits include assessment and management of acute and chronic conditions, medication and minor procedural care (ex: suture removals), chronic disease monitoring, and preventive follow-up. Nurses also support maternal-child health, substance use treatment, and care management through patient education and coordination. Care is delivered through a team-based, interdisciplinary model focused on whole-person care and improved patient outcomes. Required Qualifications BSN Graduates: Baccalaureate Degree in Nursing from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) 30 days before start date. ADN Graduates: Associate Degree in Nursing from an accredited program 30 days before the start date. JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. The Knight Cancer Institute Outpatient Clinics deliver comprehensive care to adult ambulatory patients diagnosed with a wide range of cancers, including solid tumors and benign hematology. We are seeking an Advanced Practice Provider to join our Thoracic, Head & Neck Program. Function/Duties of Position Position Summary OHSU classification: Clinical Associate APP , represented by the Oregon Nurses Association & Advance Practice United, subject to the active collective bargaining agreement. This Clinical Associate-APP provides care in conjunction with a team comprised of physicians, APPs, RN coordinators, and other allied health professionals. APPs may practice both independently or in a shared-care model with physicians, as determined by the needs of the team.
