This role performs clinical assessments, gathers patient information, collaborates with providers, and ensures appropriate level-of-care placement while maintaining compliance with regulatory and standards. The intake coordinator serves as the key point of contact for patients, families, referral sources and internal care teams. This individual will also be responsible for marketing and conducting outreach and educational visits to local healthcare facilities to increase awareness of the organization’s psychiatric services. Responsibilities Assesses the needs of walk-in and phone callers and ensures their referral to the service or resource that best addresses those needs. Coordinates and prioritizes in-patient bed reservations and placement; ensure the seamless admission of the patient to the appropriate level of care. Is responsible for advising, informing and assisting the physician and the physician's office before and during hospitalization; informs them of hospital policies and procedures related to their patient's stay. EXPERIENCE AND REQUIRED SKILLS: · Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse. · Chronic/acute hemodialysis experience (preferred).
