Overview Under direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit. The duties require the use of tact and judgment in the care and treatment of patients. May assist in the supervision of non-professional subordinates. Is cognizant of philosophy, standards, objectives and policies of the Nursing Department. Performs related duties as required. Responsibilities Demonstrates proficient knowledge in the treatment of patients requiring nursing care. Uses available diagnostic results and other information to make an accurate evaluation. Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process. Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted. Is considerate of residents' privacy during the evaluation process. Contributes to an individualized and appropriate plan of care delineating measurable patient care goals Contributes to the plan of care making necessary changes as the condition of the patient indicates. Bases care decisions on resident needs and care priorities. Monitors patient outcomes. Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement. Collaborate, as appropriate, with Care Manager, physician, and other clinical disciplines in planning interventions to meet resident and family needs in a timely manner. Qualifications Education: Graduation from an accredited school of practical nursing or completion of equivalent study in an approved program in professional nursing. Experience: Minimum one (1) year clinical experience preferred. Knowledge and Skills: Ability to work in surroundings with degree of dust, noise, etc. without undue discomfort Ability to hear, talk, walk, stand, bend and stretch Ability to read, write and interpret documents Ability to sit for long periods of time at desk to perform job functions Ability to stand for long periods of time at required patient bedside and/or procedural areas Ability to communicate effectively with patients and/or co-workers May be exposed to mechanical/electrical/radiant/explosive/burn and chemical hazards May be required to handle, dispose of and/or identify hazardous waste and hazardous materials May be required to operate and clean equipment May be required to handle/ assist in lifting patients onto/out of exam beds/procedure stations Must be trained in and knowledgeable of all Kingsbrook/Rutland policies and procedures pertaining to hazardous waste and hazardous materials and maintain qualifications applicable to assigned work Other: Knowledge of all codes and demonstrate understanding Licenses, Certifications: Current NYS Licensure or the eligibility to be licensed to practice as a practical nurse. Description The Clinical Program Manager for ALIGN-CARE Geriatric Surgery Co-Management Program is responsible for overseeing the continuity of patient care. Patients will be 65 years or older, frail, community-based, considering surgery, assessing surgical risks and provide care plans to optimize health prior to surgery. Patients enrolled in the program may come from any part of Mount Sinai Health System; however, hospitals of care receipt will be both Mount Sinai Hospital and Mount Sinai Morningside Hospital. Responsibilities Oversees clinical operations within the program, using data and fact-based problem-solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes or recommends changes to superiors as necessary. Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities. Serves as a clinical role model and resource to the nursing staff. May include direct patient care/clinical duties including patient assessments, testing, patient education, and managing patient emergencies. Participates in the application of the nursing process to an assigned group of patients. 2. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse 3. Performs job duties under supervision of a Registered Professional Nurse. 4. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. 5. Administer IV fluids, provided completion of annual competency IV therapy training. 6. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. 7. Makes recommendations in the application of the nursing process. 8. Participates in planning, and administers direct patient care, including activities of daily living (ADL). 9. Performs point of care testing including blood glucose. 10. Performs nursing procedures as defined in the nursing procedure manual. 11. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. 12. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. 13. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. 14. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. 15. Carries out medical and nursing orders. Cannot accept verbal orders or carry out RN standing orders. 16. Makes, reports and records observations on assigned patients within the EMR. 17. Observes and reports any signs and symptoms of change in a patient’s condition to the appropriate person. 18. LPNs in the Outpatient Hemodialysis Setting An LPN can practice in the chronic hemodialysis outpatient department under the direct supervision of an RN. The LPN will participate in educational programs on a regular basis, including initial orientation and continuing in service training to care for patients with central venous access devices (CVAD). The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Assist in the procedure room and recovery area, ensuring clinical stability and emotional support for patients. Administer and monitor medications used to induce sedation in accordance with PPGNY protocols and RN scope of practice. Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with Mount Sinai Health System Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Responsibilities Responsible For Development, implementation, coordination and evaluation of clinical programs and services related to core measures. Principal Duties and Responsibilities Clinical Leadership 1. In conjunction with Nursing, Medical and Quality Leadership, coordinates core measures and clinical intervention compliance activities. 2. Assists in development and design of action plans to support compliance with measures indicators and provides support and educational resources to departments and services relative to core measures quality assessment and improvement processes. 3. Assists in establishing and implementing short and long term goals, objectives, policies and operating procedures for core measures programs. 4. Provides coordination and leadership in the execution of day to day program activities, as appropriate to program objectives and areas of expertise. 5. The Nurse-Family Partnership (NFP) Nurse establishes a trusting, therapeutic one-on-one relationship with low-income, first-time mothers who voluntarily consent to home visits throughout pregnancy and until the child reaches two years of age. The nurse conducts comprehensive assessments of the family’s physical, emotional, social, and environmental needs and provides counseling, health education, referrals, and case management related to prenatal and postpartum care, nutrition, parenting, child health and development, family planning, and life-course development. The nurse-client partnership is the foundation for change, as the nurse supports clients in clarifying personal goals for themselves and their children and developing the skills needed to achieve those goals.
