Job Summary The mission of the Certified Medical Assistant (CMA) is to support the health and wellness center’s clinical and clerical operations. As directed by the provider and Clinical Operations Manager, the MA assists the provider and performs appropriate tests and procedures. The CMA is responsible for administrative tasks for the health and wellness center and may assist the Patient Advocate in employee/patient engagement and outreach. Essential Duties And Responsibilities Clinical: Interviews patients and measures vital signs, such as pulse rate, temperature, blood pressure, weight, and height, BMI calculation, pulse oximeter and records information in the patient’s electronic medical record. Performs clinical procedures including, but not limited to: injections, Electrocardiography (EKG), routine U/A, dipstick, capillary blood draw, Cholestech, Glucometer, and other medical tests and treatments under the direction of the provider. Proficient in spirometry venous labs draws, centrifuge Dispense medications as directed by the physician. Proficient in the use and maintenance of Automated Eternal Defibrillator (AED) Prepares treatment rooms and patients for examination Cleans and sterilizes instruments as appropriate Clerical: Performs administrative tasks, completes appropriate forms, and manages the front desk/check in of patients Answers phones/phone triage and scheduling appointments Responsible for compliance and regulatory reporting Maintain clinic and patient records and telephone encounters Inventories and orders medical supplies, materials, and medications Follow up with center patients who miss appointments Schedule external appointments/referrals and make follow-up phone calls to patients regarding scheduling Ensure completion of all services ordered by the physician such that all reports, consult notes, and follow ups are in the chart for the physician to review Under the direction of the Patient Advocate, periodically assists with engagement/outreach functions Required Knowledge, Skills, & Abilities Required: High school diploma or equivalent (associate degree preferred) Active and unrevoked certification such as RMA, CCMA, CMA, NCMA, or NCRMA certification Candidates who do not possess Medical Assistant Certification must obtain Certification within 6 months of employment and have graduated from an accredited Medical Assisting program and twelve months of direct clinical patient care experience in a healthcare setting. Current BLS certification through American Heart Association valid for at least 90 days after start date Knowledge of Internet software, Spreadsheet software, Word Processing software, and Electronic medical records Ability to communicate effectively and maintain working relationships with people from diverse backgrounds Ability to prioritize needs and plan work accordingly Knowledge of HIPAA confidentiality requirements Work Environment & Physical Requirements This job primarily operates in a medical office environment and is required to interact with patients for the majority of the workday. Overview Under direction to perform work involving the development, implementation and evaluation of the exercise and education experiences of clients referred to Phases I, II, and III Cardiac Rehabilitation and Phases II and III Pulmonary Rehabilitation. Participates in meeting education of Heart Failure patients and follow up of their progress. Participates with community events related to the CV Service Line. Responsibilities PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions) Collaborates with the Exercise Specialist to inform patients of need to schedule annual stress tests, to complete body composition measurements and a lipid profile (at M.D. discretion), to prepare client progress reports, and to schedule and complete follow-up outcomes data collection sessions. Coordinates staff responsibilities during emergency situations. Participates in a review of emergency procedures on a monthly basis. Coordinates the client's care by collaborating on an ongoing basis with all members of the health care team, including but not limited to physician, staff nurses, dietitians, respiratory therapists, social workers and Cardiopulmonary Rehab staff. Documents information in the client's chart in accordance with hospital and department policy. Counsels/educates client's and their families regarding their disease process therapy and appropriate lifestyle modification. Counseling/education sessions may be done in a group or individual session. Teaches (Phase II and III) group session on an assigned basis and (Phase I, II and III) individual sessions as needed. Develops/revises education outlines as assigned. Demonstrates Phase I, II, and III exercises. Supervises and evaluates client's heart rate, rhythm, blood pressure and symptomatic responses to prescribed exercises. Communicates observations to other staff members on an ongoing basis. Develops at the time of referral (Phase I) or orientation (Phases II and III) an exercise and education care plan for each client. Reviews the care plan each time the client is seen and updates as needed. Communicates the care plan to other staff members both verbally and in writing on an ongoing basis. Evaluates program effectiveness through staff interaction, patient surveys and needs assessment on an ongoing basis. Make recommendations to the Team Leader of Cardiopulmonary Rehabilitation regarding program development. Participates in new program development and implementation. Interviews clients at the time of referral (Phase I) or orientation (Phases II or III) and completes the appropriate forms and formulates a nursing assessment. Communicates this assessment to other staff members on an ongoing basis. Provides the Team Leader of Cardiopulmonary Rehab with proper billing information on a daily basis in accordance with established departmental procedures. Review and process all wound score deviations, documenting any action and follow up. Review and process vital sign alert reports; document follow-up action and physician notification. Receive lab reports and assess for normality; fax lab report to the physician with signature indicating review. Scan both the reviewed labs and the fax confirmation page (showing it was sent to physician) to medical records for uploading into the patient chart. Initiate employee and patient infection reports, as necessary. Complete review of evaluation documentation and plans of care (POC). Review the data submitted to ensure accuracy with the POC; follow up on any documentation that requires correction. Process POC and verify the correct start of care date. Review comprehensive assessments that cannot be processed due to licensed professional documentation deficiencies; follow up appropriately. ** INFUSION NURSE (RN) ** Plymouth, IN Area ** SUMMARY: The Infusion Nurse reports to the Director of Nursing and is responsible for providing infusions. The infusion nurse is responsible for ensuring that care is delivered appropriately, and the standards of quality are adhered to. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Perform care as defined by client’s individual Plan of Care and within scope of state’s nurse practice act. • Monitor patient’s condition and notify appropriate personnel of any changes. • Provide those services requiring substantial and specialized nursing skill. • Assures proper maintenance of clinical records in compliance with local, state and federal laws. • Counsels the client and family in meeting nursing and related needs. • Preforms all other duties as assigned.
