Ensures that care plan is carried out which demonstrates familiarity with criteria to establish priorities specific to anticipated outcomes. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Understands and performs within scope of practice. Obtains vital signs: BP, temp, pulse, height & weight accurately and completely as required. Documents appropriately in EHR. Reviews and updates medication list and accurately documents known allergies in the Electronic Health Record (EHR). Demonstrates an understanding of prescription control and prescription refill procedures. Accurately documents allergies in EHR. Appropriately enters orders and schedules appointments in the Practice Management System (PMS) and EHR. Provides referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR. Appropriately adheres to universal safety precautions when administering medications and disposal of medical waste. Maintains proficiency with department specific equipment, troubleshoots and reports malfunctions promptly Facilitates transition of care to UCC/Hospital Demonstrates flexibility with various work schedules. All other duties as assigned Recognizes and performs duties which need to be performed although not directly assigned. Provide scribing services. Education, Certification, Computer & Training Requirements: High School Diploma or equivalent, required. Graduated from an accredited medical assistant or phlebotomy program, required. 0-1 year related work experience, required. 2-4 years related work experience, preferred.
