The Case Manager, RN directly guides participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination. A Bachelor’s degree in Nursing is strongly preferred, as is prior patient care experience ideally emergency room nursing and comfort with electronic medical record systems (EMR) like EPIC. In this role, you will work 11a-11:30p and every other weekend and rotating holidays. The work week is 36 hours per week. About the Role: We are seeking a compassionate and reliable caregiver to provide respite care for two elderly parents (both 85 years old) residing with their daughter in Armonk, NY. The caregiver will support daily activities, ensure comfort, and foster a safe and engaging environment. Location: Armonk, NY Schedule: 4 days per week, 11:00 AM – 2:00 PM or 11:00 AM – 3:00 PM Responsibilities: Accompanying on walks in the park Escorting to doctor appointments Assisting with laundry Performing light housekeeping tasks Handling trash removal Qualifications: Prior caregiving experience preferred (elderly care experience is a plus) Patient, empathetic, and attentive to seniors’ needs Ability to assist with mobility and provide companionship Reliable and punctual Compensation: Competitive, based on experience.
