Essential Duties & Responsibilities of the Registered Nurse: Obtain changes in physician’s orders as indicated by resident’s progress or lack of progress Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated. Maintain clean, orderly treatment cart and restock treatment supplies. Follow Infection Control policies and procedures. Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures Document pressure sore status weekly or more often as condition changes. Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents. Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home. Participate in Quality Improvement activities, as assigned. Other special projects and duties, as assigned. Job Requirements of Registered Nurse: Registered Nurse current state license in Florida One (1) year related nursing experience preferred Must have a current/active CPR certification. Excellent technical, assessment, and documentation skills. Effective verbal and written English communication skills. Highest level of professionalism with the ability to maintain confidentiality. Ability to communicate at all levels of organization and work well within a team environment in support of company objectives. Customer service oriented with the ability to work well under pressure. Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis. Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics. Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned. Routinely turn and position residents, as necessary. Observe residents carefully and report changes in condition to Charge Nurse. Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care. Provide adequate hydration to residents at regular intervals each day according to specific resident needs. Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves. Keep an accurate record of food intake and output, as directed. Be alert for proper ventilation, temperature, light and noise control. Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed. Promptly answer call lights and assist with other resident needs. Assist in the orientation and training of other staff, as requested. Requirements/Qualifications of Certified Nursing Assistant: Current Nursing Assistant certification from state of Florida. Current/active CPR certification, preferred. At least one (1) year Skilled Nursing experience preferred. Effective verbal and written English communication skills. Responsibilities Reports To: Office Manager Position Objective: The Medical Assistant is responsible for providing both clinical and administrative support to ensure the delivery of safe, high-quality, patient-centered care. Working under the direction of a Physician, Physician Assistant, or APRN, the Medical Assistant prepares patients for examinations, performs clinical procedures within the scope of practice, assists providers during patient visits, and supports front office operations. The MA plays an active role in supporting Medicare Advantage patients within a value-based care model that prioritizes quality outcomes, and preventive care. Essential Responsibilities: Patient Care: Welcome patients and prepare them for provider examinations by obtaining and documenting patient history, chief complaint, current medications, allergies, medical history, surgical history, family history, social history, preventive care information, and other relevant clinical information in the electronic medical record (EMR). Measure, document, and report vital signs and other clinical observations. Prepare patients and examination rooms for provider visits. Assist providers during examinations, treatments, and in-office procedures. Explain provider instructions, treatment plans, medications, and follow-up care to patients as directed. Provide compassionate, professional service that promotes a positive patient experience. Clinical Procedures: Administer oral and injectable medications, immunizations, and treatments as directed by the provider and permitted by state regulations. Assist with wound care, dressing changes, bandaging, irrigation of eyes and ears, and other clinical procedures. Initiate and monitor IV infusions when appropriately trained and permitted by state regulations. Perform phlebotomy and prepare specimens for laboratory testing. Perform CLIA-waived laboratory testing in accordance with established procedures. Conduct in-office diagnostic testing, including but not limited to: Electrocardiograms (EKGs) Vision screenings Pulse oximetry Peak flow testing Oxygen administration as ordered Inhalation treatments Spirometry Perform PT/INR testing using approved equipment (CoaguChek/Coag-Sense) and glucose testing using a glucometer. Complete outbound and inbound patient communications, including: Appointment reminders Medication reminders Transitional Care Management (TCM) — Medicare-supported outreach within 2 days of hospital or facility discharge to reduce readmissions and support safe care transitions Preventive care outreach Support Annual Wellness Visits (AWVs) by preparing Health Risk Assessments (HRAs), updating preventive care records, and ensuring completion of required documentation. Assist in patient outreach, scheduling of preventive screenings, and documentation of completed services. Care coordination activities Maintain patient medical records in accordance with organizational policies and HIPAA requirements.
