Position Summary: The Float LPN Wellness Nurse assists and supports the Resident Services Director with the overall implementation, delivery, and coordination of nursing services in the community. The Float LPN will travel between Maplewood at Twinsburg, Maplewood at Chardon, and Maplewood at Cuyahoga Falls. Key Responsibilities: Medication administration Direct care and service to residents Teaching and wellness counseling Health Promotion Participation in assessments under the supervision of the Resident Services Director (RN) Support of residents and families Oversight of Certified Nursing Assistants Training of associates and auditing of care Completing monthly in-service training as required Education/Experience/Licensure/Certification: Licensed Practical Nurse currently licensed to practice in the State Two years full time or full time equivalent clinical experience Experience and/or special interest working with older adults Able to perform tasks which may be physically demanding such as pushing, bending and lifting up to 50lbs. The RN would be on call from Friday at 4:30 pm until Monday at 8:00 am (eastern). The RN would be responsible to cover any as-needed visits that arise, as well as scheduled weekend visits including admission visits. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The Nurse will be responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration required.
