187593 Job Description Performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team. Please note: The 'minimum’ rate and 'maximum’ rate refers to a 'per-visit' rate for regular assignments in this role. Voluntary work opportunities, outside the normal scope of duty, may pay an alternate ‘per-visit' rate, depending on multiple factors (region, type of visit, discipline, etc.), and may be offered on an occasional, as-needed basis. There is an hourly rate paid for certain administrative tasks (orientation, training, meetings, etc.), which is $30.00, following completion of new hire orientation. Job Responsibility Conducts pre-admission screening and initial nursing health assessment on new patients; assess body systems (cardiac, vascular, neurological, and gastrointestinal, etc.); develops nursing plan of care and goals based on patients needs. Provides nursing care to patients to include but not limited to supervising diet, medication, activity level of client, supervising safety in the home and activities of daily living. Evaluates the provision of nursing care to patients including but not limited to Evaluation of physical, therapy, occupational therapy, speech, therapy, nutrition counseling and respiratory therapy, Contracting agency nurses. Trains and evaluates home health aides, homemakers and personal care workers in the patient’s home, recommending personnel changes to the administration of Hospice as is necessary. Determines the need for, and recommends the rental or purchase of medical equipment and/or supplies for patients. Provides on-going evaluation of equipment for defects, repairs or additional needs. Recommends to the Hospice IDG (Interdisciplinary Group) when physical therapy, occupational therapy, speech therapy, nutritional counseling and respiratory therapy are warranted, as per his/her clinical assessment; requests physician written orders for changes in the plan of care as is appropriate; reports to and consults regarding patient status; participates in on-going orientation and in-service training offered both within and outside the Hospice. Participates in on-call coverage system on a rotating basis; serves as a liaison in maintaining the Hospice patients Plan of Care while inpatient; refers major or recurrent problems to the Hospice supervising nurse. Participates in the on-going Quality Improvement Program. Performs related duties as required. Job Qualification Graduate from an accredited School of Nursing. Bachelor’s Degree in Nursing, preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date. Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed. 2 + years of nursing experience: med/surge, hospice, home care preferred. Minimum one year of experience in acute care – med/surg strongly preferred. Experience in inpatient facilities and hospice care. This is a Field position. May need to see Pediatric patients. Description The Medical Assistant III provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Focus of tasks include basic rooming of patient, EKGs, Point of Care Testing and documentation in EPIC. In addition to the MA II skills, the MA III will be responsible to train staff, follow quality assurance guidelines, staff leadership and/or significant experience/skills in a particular specialty such as endoscopy, etc. The MA III should have at least 3+ years experience in a specialized area such as phlebotomy, pulmonary testing, surgery or cardiology . Responsibilities 1. Fulfills patient care responsibilities as assigned which may include: performing venipuncture and/or EKGs, checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring and dressing, as well as collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. During endoscopies and colonoscopies, Medical Assistant directly assists physician with duties such as, biopsies, snare manipulation, and patient positioning. 2. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. 3. Enters orders into the electronic medical record EPIC system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. 4. Inventories, orders or re-stocks medical supplies as applicable. 5. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. Additional duties include the maintenance, cleaning, processing, and storage of scopes for endoscopes and colonoscopes. 6. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. 7. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. 8. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. 9. Assists in the maintenance of medical charts. 10. May be required to perform proficient phlebotomy duties. 11. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. 12. May check and verify and update insurance information utilizing internal or web sources. May perform and verify pre-certification for procedures as necessary. 13. Provides training and mentoring to less experienced Medical Assistant staff. 14. Performs other duties as assigned. Job Responsibility Conducts pre-admission screening and initial nursing health assessment on new patients; assess body systems (cardiac, vascular, neurological, and gastrointestinal, etc.); develops nursing plan of care and goals based on patients needs. Provides nursing care to patients to include but not limited to supervising diet, medication, activity level of client, supervising safety in the home and activities of daily living. Evaluates the provision of nursing care to patients including but not limited to Evaluation of physical, therapy, occupational therapy, speech, therapy, nutrition counseling and respiratory therapy, Contracting agency nurses. Trains and evaluates home health aides, homemakers and personal care workers in the patient’s home, recommending personnel changes to the administration of Hospice as is necessary. Determines the need for, and recommends the rental or purchase of medical equipment and/or supplies for patients. Provides on-going evaluation of equipment for defects, repairs or additional needs. Recommends to the Hospice IDG (Interdisciplinary Group) when physical therapy, occupational therapy, speech therapy, nutritional counseling and respiratory therapy are warranted, as per his/her clinical assessment; requests physician written orders for changes in the plan of care as is appropriate; reports to and consults regarding patient status; participates in on-going orientation and in-service training offered both within and outside the Hospice. Participates in on-call coverage system on a rotating basis; serves as a liaison in maintaining the Hospice patients Plan of Care while inpatient; refers major or recurrent problems to the Hospice supervising nurse. Participates in the on-going Quality Improvement Program. Physical Demands are constant – standing and walking; frequent – stooping, bending, stretching, lifting, pulling, pushing, and turning; lifting of objects from 10 lbs. to 150 lbs. and pushes objects up to 200 lbs.; occasional – sitting; lifting with assistance objects greater than 150 lbs. Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed. 2 + years of nursing experience: med/surge, hospice, home care preferred. Minimum one year of experience in acute care – med/surg strongly preferred. Experience in community health, hospice, and home care preferred. This is a Field position.
