Description Patient Care Associate MSH GP9C Med/Surg FT Nights EOW As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Description The Assistant Nurse Manager Vascular Thoracic Night Shift position is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Assists in the validation of staff members round hourly on all patients by direct observation and patient feedback. 5. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6. Conducts rounding of all patients on a daily basis when Nurse Manager is away and as assigned 7. JOB DESCRIPTION Distinguishing Characteristics Graduate Nurses (T&RC) and Registered Nurses 1 (T&RC) perform similar nursing tasks and activities; coordinate and provide nursing care to individuals in State University of New York teaching and research center hospitals; administer treatments and nursing procedures within established guidelines and as prescribed by licensed health care providers; and consult on nursing matters with treatment teams and other disciplines. Levels in the series are primarily distinguished by scope and independence of operation, which is based on incumbents’ credentials (i.e., possession of a limited permit as a graduate nurse or eligible to enter examination for licensure as a registered professional nurse; or possession of a license and current registration as a registered professional nurse in New York State). Graduate Nurse (T&RC): entry level; possesses a limited permit as a graduate nurse or is eligible to enter examination for licensure as a registered professional nurse in New York State; under the close supervision of a higher-level registered nurse, provides direct nursing care to individuals in accordance with Article 139 of the Education Law. Illustrative Duties Graduate Nurse (T&RC) Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Carries out treatment procedures, administers medications, and monitors individuals’ conditions. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Participates in the development, implementation, and revision of individual care plans as a member of a multidisciplinary treatment team. Triages individuals by inquiring about illnesses, injuries, and symptoms, and records findings and observations. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. May perform the full range of supervisory duties over Graduate Nurses (T&RC), Teaching & Research Center Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors 1 (T&RC); and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Registered Nurse 2 (T&RC) Positions perform duties in each of the three functional areas described below. Direct Individual Care: Provides nursing care to individuals in an acute care environment requiring complex nursing skills; uses advanced knowledge and skill in nursing assessment, planning, implementation, and evaluation activities. Observes and responds to individual emergency situations, and sets priorities for care based on individual acuity, preferences, and the possibility of effective intervention. Description Position Title: Licensed Practical Nurse-Institute for Advanced Medicine-Mount Sinai West-Full Time-Days Position Summary: Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse. Responsibilities A. Clinical/Technical/Service: Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse Performs job duties under supervision of a Registered Professional Nurse. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. Administer IV fluids, provided completion of annual competency IV therapy training. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. Makes recommendations in the application of the nursing process. Participates in planning, and administers direct patient care, including activities of daily living (ADL). Performs point of care testing including blood glucose. Performs nursing procedures as defined in the nursing procedure manual. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). Staffing Coordination & Assignment Support Collaborates with Nursing Leadership to ensure safe staffing, skill mix, and equitable assignments Manages real-time staffing challenges including unplanned absences Ensures appropriate coverage for high-risk patients, including constant observation (1:1) Communicates staffing updates and escalates trends using data-supported recommendations Responds timely to Protests of Assignment (POA) utilizing the Management Response form as outlined per policy Patient Experience & Service Recovery Conducts patient experience rounds during assigned coverage Assesses responsiveness, communication, comfort, pain management, and understanding of care Initiates real-time service recovery and documents interventions Reviews patient experience data and addresses unit-level trends Validates patient experience tactics (i.e. Bedside Shift Report, Hourly Rounding) through direct observation Provides real-time coaching and reinforcement of best practices Escalates repeated non-compliance patterns through appropriate channels Throughput, Patient Flow & Length of Stay Leads real-time Emergency Department patient flow from arrival through dispositionMonitors ED census, arrivals, holds, and throughput metrics Identifies and removes barriers related to diagnostics, consults, transport, and discharge readiness. Collaborates with physicians, case management, ancillary services, and receiving units to reduce delays and ED boarding. Balances throughput decisions with patient safety, staffing capacity, and patient experience. Responds to patient surges, EMS volume, disasters, staffing shortages, and system disruptions. Lead throughput rounds when boarding inpatients holds. Ensures clarity of care plans, disposition goals, and identified barriers. Ensures timely ICU downgrades and appropriate utilization of critical care services. Tracks unresolved throughput issues and escalates delays impacting safety or Emergency Department length of stay. Quality, Patient Safety & Performance Improvement Monitors key performance indicators including medication scanning, hand hygiene, restraints, falls, and observation compliance Collaborates with Risk/PI on occurrence reviews and follow-up actions Identifies trends, near misses, and opportunities for improvement Conduct real-time Root Cause Analysis in response to unexpected patient safety events as needed Hospital Acquired Conditions & Harm Prevention Supports prevention strategies for all Hospital Acquired Conditions and patient safety outcomes Validates prevention practices and timely interventions related to patient quality and safety outcomes Partners with Infection Prevention, Wound Care, Pharmacy, and Quality teams Environment of Care & Regulatory Readiness Conducts routine environment of care and safety rounds and documents accordingly Maintains continuous readiness for regulatory and accreditation standards Works with Emergency Management and Environment of Care team to develop attainable action plans for environmental risks Identifies and escalates environmental, equipment, and safety risks Staff Engagement Conducts employee rounding across all shifts Provides real-time coaching, feedback, and support Facilitates safety huddles and reinforces quality and safety priorities Escalates recurring performance or operational concerns Leads or supports pre-shift huddles and unit communication Encourages staff participation in shared governance councils Supports ANCC initiatives Collaboration with NPD, HR & Employee Health Supports compliance with mandatory education, competencies, and certifications Collaborates with Nursing Professional Development, Human Resources, and Employee Health on annual employee requirements Tracks compliance and escalates gaps for remediation Qualifications Bachelor of Science in Nursing (BSN) required; Master’s degree preferred Active RN license in NY Minimum of 3–5 years nurisng leadership experience, preferably in ED Strong communication, critical thinking, and operational leadership skills National Certification in Nursing Leadership or clinical specialty preferred
