Participates in the development of a patient-specific care plan. Provides direct patient care in patient homes including administering medications and treatments as prescribed. Understands and applies knowledge in edema/lymphedema management, venipuncture, management of PICC lines, implanted ports, post-surgical drains, and pulmonary and abdominal drains. Provides focused systems and wound assessment. Reports acute medical problems and condition changes to the case manager and/or attending physician, as directed. Demonstrates proficiency in communicating effectively with individuals diagnosed with mental and behavioral disorders, and to operate efficiently in an independent home setting. Documents patient care in health record according to department standards. Delegates, prioritizes, requests and accepts guidance and assistance from others to assure delivery of care in a safe and timely manner. Ensures own professional effectiveness through continued education and professional development. Assists in the transporting of patients, specimens, supplies and equipment and in the maintenance and stocking of supplies and equipment to patient homes. May triage patient phone calls, counsel patients, providing patient education, taking patient histories, administering prescribed medications, changing dressings, cleaning wounds and monitoring patient vital signs. Teaches the patient and caregivers about disease management, in accordance with the plan of care. Performs other duties as assigned. Qualifications Education Required: Accredited State Board of Nursing LPN Program Experience Minimum of 1 year Preferred: clinical experience as an LPN and Preferred: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs Credentials Required: Upon Hire Licensed Practical Nurse - Oregon Required: Upon Hire Basic Life Support Required: Upon Hire Driver's License and proof of insurance Skills Basic computer skills required including familiarity with MS Office applications. (Preferred) Working Conditions Lifting Patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices (NIOSH). Fine motor skills to be able to grasp and control medical equipment and perform precise procedures. Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts). Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally. Ability to move around area with frequent sitting. Bending/ stooping/ squatting/ reaching/ kneeling frequently. Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently. Environmental Conditions Exposure to biohazard, body fluids and airborne particles. Must be able to complete tasks in a noisy environment. Assists in the transporting of patients, specimens, supplies and equipment and in the maintenance and stocking of supplies and equipment to patient homes. May triage patient phone calls, counsel patients, providing patient education, taking patient histories, administering prescribed medications, changing dressings, cleaning wounds and monitoring patient vital signs. Teaches the patient and caregivers about disease management, in accordance with the plan of care. Performs other duties as assigned. Effectively communicates with patient/family the POC and progression of such. Keeps the patient informed ongoing. Principal Responsibilities: This position reports to the Home Health Director Understands and adheres to established policies and procedures. Implements the nursing care plan for each patient. Provides nursing services, treatments, and diagnostic and preventive procedures as assigned. Initiates preventive and rehabilitative nursing procedures as appropriate. Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Evaluates with registered nurses the effectiveness of the LPN’s/LVN’s nursing service to the patient and family under the guidance of the registered nurse. Maintains accurate and complete records of observations, treatments, and care of patients. Participates in medical record audit as assigned. Attends staff meetings, patient care conferences, and in-servicess as scheduled. Takes on-call duty, nights, weekends, and holidays as assigned. Is responsible for: Submitting any changes in schedule to the Director of Clinical Services/Clinical Manager daily. Participating in patient care conferences to discuss the need for the involvement of other members of the health team, such as physical therapists or speech-language pathologists Prepares clinical and progress notes. Assists the physician and RN in performing specialized procedures. Prepares equipment and materials for treatments. Observe the aseptic technique as required. Assists the patient in learning appropriate self-care techniques.
