Essential Functions Patient Care Provide direct patient care in accordance with the state Nurse Practice Act, including specialized duties related to end-of-life care. Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique. Communication & Coordination Document all care accurately and timely in alignment with the plan of care. Participate in care coordination and promptly report needs requiring involvement from other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Care Manager.
