Document all care, observations, and evaluations accurately and submit documentation timely. Maintain strong communication with clients, families, and the care team. Adhere to all federal, state, and local regulations, as well as Trinity Health’s Integrity Program and Code of Ethics. Demonstrate commitment to quality improvement and client‑centered outcomes. Minimum Qualifications Graduate of an accredited or state‑approved practical nursing program. Current LPN licensure in the state of practice. At least 1 year of acute care experience required; homecare experience preferred . Successful completion of NAPNES or equivalent medication exam if administering medications. Strong interpersonal and communication skills. Reliable transportation and a valid driver’s license.
